Press Needles Reduce Atrial Fibrillation Recurrence After Radiofrequency Ablation:A Randomized Controlled Clinical Trial

Objective: To observe the recurrence rate of AF after radiofrequency ablation in patients with atrial fibrillation by using pinprick,the exploratory demonstration that pinprick therapy can reduce AF recurrence after radiofrequency ablation and is safe and green with high acceptance by patients may be considered as an intervention for preventing and delaying recurrence after radiofrequency ablationMethods: This trial started in January 2020 and ended in January 2022.All 40 cases were recruited from the Department of Cardiology,Tianjin Chest Hospital,Tianjin First Central Hospital,and were treated with acupuncture at the First Affiliated Hospital of Tianjin University of Chinese medicine.All patients had to meet the diagnostic criteria of traditional Chinese and Western medicine for AF,meet the inclusion criteria,patients and family members were fully informed about the relevant conditions of this trial and signed an informed consent.A random number table method was used to randomize the 40 patients into the press needle group,the control group,and each group had 20 patients.All patients were normally treated with drugs such as antiarrhythmic,anticoagulation,acid suppression and gastric protection after radiofrequency ablation and received acupuncture within 1 week after surgery.Needle sets the needle sets were selected with huatou brand disposable intradermal needles(0.22×1.5mm)in bilateral Neiguan,Xinsu,and Shenmen acupoints.Control group were selected with huatou brand disposable millineedles(0.3×40 mm),taking non crypts 1,2and 3.(non acupoint 1: midpoint of the connection between the Shentang and Tianzong,non acupoint 2: midpoint of the connection between the Quze and Qingling,and non acupoint 3:midpoint of the connection between the Tianquan and Qingling),retaining the needle for 30 min.The 2 groups were all treated for 12 weeks,with 2 treatments per week for the first 6weeks and 1 treatment per week for the second 6 weeks.A 7-day long-range ECG monitoring was worn for the patient after the end of treatment.To evaluate the recurrence rate of AF 3months after radiofrequency ablation in patients,the impact of AF on quality of life(AFEQT)before and after treatment,the sleep quality score(PSQI)before and after treatment,and to record the occurrence of adverse effects and medication use.Results:1.Loss of ambulation: a total of 40 patients in both groups completed the study observation during the course of the trial,and no loss of ambulation occurred.2.Adverse event record: during the course of the study,one patient in the control group experienced local bluish violet color on acupuncture,pacified the patient,managed promptly,did not experience other discomfort,and completed treatment smoothly.3.Main outcome measures: AF recurrence rates at three months after the procedure were analyzed for both groups,with one recurrence in the pinprick group and 19 non recurrences,for a recurrence rate of 5%.There were 3 recurrences in the control group and 17 without recurrence,for a recurrence rate of 15%(P = 0.60 > 0.05)by chi square test,there was no significant difference between the recurrence rates of the two groups.Comparison of the base case between patients who relapsed and those who did not,by independent samples t-test,revealed no statistically significant differences between patients who relapsed and those who did(P > 0.05)in age,BMI,LAD,LVEF;By rank sum test,relapsed and non relapsed patients were statistically significant in terms of disease duration(P < 0.05)and did not differ significantly in terms of CHA2DS2-VASc score.4.Comparison of AFEQT: A paired t-test was used to evaluate the differences between the before and after treatment in the press needle group and the control group,with P < 0.05 for the press needle group and P < 0.05 for the control group,and the AFEQT scores of the two patients after treatment were lower than those before treatment.The scores of patients in the two groups before treatment were found to be not statistically significant(P > 0.05)and comparable using the independent samples t-test.Points were statistically significant(P < 0.05)between the two groups after treatment,and AFEQT scores were smaller in the press needle group than in the control group.Comparison of PSQI: Using nonparametric tests to compare the before and after treatment between the press needle group and the control group,the PSQI total score of the press needle group was p < 0.05,the control group P > 0.05,and the PSQI total score of the press needle group after treatment was smaller than that before treatment,whereas there was no significant difference between the control group and that before treatment.The P values of all 7 dimensions of PSQI in press needle group were < 0,05,statistically significant;All P values in the control group were > 0.05,not statistically significant;The press acupuncture group differed before and after treatment in terms of hypnotic drugs(P < 0.05)and in terms of the other 6 dimensions(P < 0.01);There were no significant differences between the control group before and after treatment in the 7 dimensions(P > 0.05).Using nonparametric tests,we found that the PSQI total scores and scores of the 7 dimensions were not statistically significant(all P > 0.05)and comparable between the 2 groups before treatment;Post treatment PSQI,time to fall asleep,sleep quality,hypnotic drugs,sleep efficiency,daytime dysfunction had P < 0.05,and the pinprick component scores were smaller than those of the control group,with P > 0.05,but not statistically significant,in terms of sleep time,sleep disturbance.Conclusion:1.Press needle at Neiguan,Shenmen,and Xinyu acupoints did not significantly decrease the recurrence of AF after radiofrequency ablation.2.Press needle therapy can improve the quality of life of patients with AF after radiofrequency ablation,improve the quality of sleep of patients and is green and safe,easy to operate,and have high patient compliance.
