Study On Medication Rules Of Traditional Chinese Medicine In Treating Hyperuricemia Based On Data Mining

Objective:1.To explore the syndrome differentiation and medication ru les of TCM in the treatment of hyperuricemia through data mining of literature.2.Through expl oring and collating the clinical medication of patients with damp-heat and blood stasis hyperuricemia in the endocrinology Department of our hospital,and combin ing with the clinical med ication experience of our tutor,the law of prescription medicat ion in the treatment of this disease in the endocrinology Department was analyzed,providing more TCM treatment ideas and scientific basis for the clinical treatment o f hyperuricemia.Methods:Study 1: Search the CNKI,CBM,Wan Fan g,VIP,Pub Med,The Cochrane Library and Embase from inception to November 2021 for RCT literature on traditional Chinese medicine in the treatment of hyperuricemia.The information extracted from the literatur e meeting the inclusion and exclusion criteria,including the number of cases,syndrome differentiation,treatment methods and prescri ption composition,was standardized an d entered into Excel software to establish a database for collati on and preliminary analysis.The statistical results were imported into the traditional Chinese medicine inheritance calculation platform(V3.0)for classification and analysis,and the common clinical syndromes and medication rules of hyperuricemia were summarized.Study 2: The effective prescriptions of damp-heat and blood stasis type hyperuricemia diagnosed in the Endocrinology Department of the First Affiliated Hospital of Heilongjiang University of Tradi tional Chinese Medicine from September 2020 to Dec ember 2021 were collected.The basic information,diagnosis and treatme nt inform ation,Western medicine diagnosis,traditional Chinese medicine diagnosis,tongue coating,pulse condition,prescription medication and other information of patients were input into Excel software,and then imported into the “data management” system of traditional Chinese medicine inheritance calculation platform(V3.0).The“data analysis” module was used to statistically analyze the characteristics of the disease,the frequenc y,efficacy and meridian attribution of traditional Chinese medicine.T he prescription medicat ion rules of damp-heat and blood stasis type hyperuricemia in our hospital were summarized.Results:Study 1:1.TCM syndrome differentiation:There are 17 TCM syndromes in 129 included literatures.The top five TCM syndrome types were damp-heat accumulation syndrome(19.03%),phlegm-dampness block syndrome(18.64%),spleen and kidney deficiency s y ndrome(15.27%),ph legm-turbidity and blood stasis syndrome(12.76%)and d amp-heat and blood stasis syndrome(11.69%).2.Classification of co mmonly used drug efficacy: a total of 18 kinds,of which the most used are diuresis and dampness drugs(24.52%),heat clearing drugs(18.83%),tonic drugs(17.69%),blood-activating and stasis-removing drugs(12.08%),wind dampness removing drugs(7.70%).3.The meridian attribution of traditional Chinese medicine:liv er meridian(21290 times),spleen meridian(21232 times),stomach meridian(18680 times),lung meridian(16554 time s),kidney meridian(14567 times),bladder meridian(8657 times),heart meridian(8440 times),gallbladder meridian(6986 times),small intes tine meridian(2647 times),large intestine meridi an(2544 times),pericardium meridian(1009 times),triple energi zer meridian(57 times).4.The use of high frequency drugs of each syndrome type: the common drugs are Tuckahoe,Bixie,Plantain Seed,Coix Seed,Achyranthes,Clematic Root.The characteri st ic drugs used in damp-heat accumulation syndrome include Atractylodis Rhizoma,Corte x Phellodendri,Loosestrife,Scutellaria Baicalensis and Polyporus;The characteri stic drugs used in phlegm-dampness block syndrome include Atractylodes Macrocephala,Li corice,Alisma orientalis,Poria,Codonopsis Pilosula,Tangerine Peel and Pinelliae Rhizoma;the characteristic drugs used in spleen and kidney deficiency syndrome include Astragalus Membranaceus,Epimedium,Clematic Root,Mulberry Parasitic,Eucommia;the characteristic drugs used in the syndrome of phlegm-turbidity and blood stasis are Salvia Miltiorrhiza,Shanci Mushroom,Wang Buliuxing and Jixueteng;the characteristic drugs used for damp-heat and blood stasis syndrome are Rhubarb and Polygonum Cuspidatum.5.Extract new p res cription combination: prescription one: Bixie,Tuckahoe,Coix Seed,Astragalus,Epimedium,Plantain Seed;prescription 2:Licorice,Pinellia,Poria,Atractylodes,Chenpi,Alisma;prescription 3:Tuckahoe,Bixie,Shanci Mushroom,Achyranthes,Clematic Root,Salvia Miltiorrhiza;prescrip tion 4: Coix Seed,Tu ckahoe,Bixie,Cortex Phellodendri,Salvia Miltiorrhiza,Polygonum Cuspidatum;prescription 5: Coix Seed,Cortex Phellodendri,Plantain Seed,Tuckahoe,Scutellaria Baicalens is,Bixie.Study 2:1.Pathogenic characteristics: A total of 92 valid cases were collected in this study,incl uding 69 males and 23 females.Age stratification: 7 persons aged 7-17,58 persons aged 18-40,24 persons aged 41-65,3 persons aged ≥ 66.2.There are 11 kinds of combined diseases in patients with hyperuricemia of damp-heat and b lood stasis type in endocr inology department of our hospital.The top five are 23 cases of type 2 diabetes,13 cases of fatty liver,12 cases of obesity,10 cases of hyperlipidemia,and 8 cases of metabolic syndrome.3.High frequency medication:Coix Seed,Bixie,Tuckaho e,Cortex Phellodendri,Plantain Seed,Coptis Chinensis,Atractylodis Rhizoma,Salvia Miltiorrhiza,Licorice,Polygonum Cuspidatum,Gardenia,Alisma.The drugs used are mainly heat-clearing drugs,followed by water-clearing and dampness-removing drugs and blood-activating and stasis-removing drugs.The selected drugs are mainly attributed to the stomach,liver and spleen meridians.Conclusion:1.Literature data mining found that the syndrome differentiation of hyperuricemia was m ainly based on damp-heat accumulation syndrome,phlegm-dampness block syndrome,spleen-kidney deficie ncy syndrom e,phlegm-turbidity and blood stasis syndrome,and damp-heat and blood stasis syndrome;the treatment methods are u sually clearing heat and r emoving dampness,drying dampness and resolving phlegm,invigorating spleen and kidney,discharging turb idity and removing blood stasis,etc.2.The traditional Chinese medicines used are mainly for diuresis and dampness;the meridians mainly enter the liver,spleen and s tomach meridians.The common drugs of different syndrome types are Tuckahoe,Bixie,Plantain Seed,Coix Seed,Ach yranthes,Clematic Root.3.The new prescription combination obtained by cluster analysis is in line with the basic pathogen esis of HUA,and can be used as the basic prescription for clinical treatment of HUA,which is worthy of further explor ation.4.The number of m ale patients with hyperuricemia of damp-heat and blood stasis type in endocrinology department of our hospital is m ore than that of female patients,the age of onset is concentrated in 18-40 years old,and most of them are com plic ated with metabolic diseases.The clinical medication is basically consistent with the high frequency medication of damp-heat and blood stasi s syndrome in literature research.On the basis of basic medication,the tutor also uses Coptis chinensis and Garde nia to improve clinical efficacy.
