Objective: To observe the therapeutic effect of blood-letting puncture and cupping combined with oral administration of Kuntai Capsule in the treatment of perimenopausal insomnia due to heart-kidney failing to meet requirements,and to explore the application value of blood-letting puncture and cupping in the treatment of perimenopausal insomnia due to heart-kidney failing to meet requirements.Methods : A total of 72 patients who met the criteria of insomnia in perimenopausal period due to cardiac-renal failure were included.The patients were divided into the control group and the treatment group using random number table method,with 36 cases in each group.The treatment group was treated with blood-letting puncture and cupping combined with oral administration of Kuntai Capsule.Pricking blood and cupping for acupoint selection: Xinshu(BL 15),Shenshu(BL 23),Ganshu(BL 18),Dazhui(CV 14),Shendao(DU 11),and Sanyinjiao(SP 6)were punctured and bled with a blood collection pen at the selected acupoints until the bleeding volume was 1–2 m L,and the cupping was pulled and sucked to the acupoints by flash fire method,and the cupping was kept for 15 minutes,and the treatment was conducted once every 2 days.Administration method of Kuntai Capsule: 4 capsules/time,3 times/day.The control group was orally administrated with Kuntai Capsule,and the administration method was the same as that in the treatment group.There was one course of treatment every4 weeks.Patients in each group were assessed with the PSQI scale,the modified KUPPERMAN scale,the TCM syndrome scale of heart-kidney incompatibility and sex hormone levels before and after treatment and the data were compared.Patients in each group were followed-up one month after the end of treatment.The PSQI scale and the TCM syndrome scale of heart-kidney incompatibility were assessed and compared with the data before treatment.Result:1.Basic data: A total of 72 patients were included in the trial,including5 cases who dropped out and 67 cases who were actually completed,including34 cases in the treatment group and 33 cases in the control group.There was no significant difference in age and course of disease between the two groups(P>0.05).2.Comparison of PSQI scores: The total scores of the two groups were significantly decreased after treatment(P<0.01),indicating that both groups could improve the sleep status of the patients and relieve insomnia symptoms.The efficacy of the treatment group was superior to that of the control group(P<0.01).As for the single score(both scores were 0 before and after the hypnotic drug treatment and no participant in the analysis),except for the sleep disorder in the control group(P>0.05),the scores of other items in the two groups were all significantly reduced(P<0.01).In the treatment group,except for the sleep time(P>0.05)and daytime dysfunction(P>0.05),the decreases in other items were significantly different from those in the control group(P<0.05),and the clinical efficacy was superior to that in the control group.3.Comparison of scores of traditional Chinese medicine(TCM)syndromes of heart-kidney failure: The total scores were significantly reduced in both groups after treatment(P<0.01),indicating that the treatment options in both groups improved the syndromes of heart-kidney failure.The effect of reducing the total scores in the treatment group was better than that in the control group,and the difference was statistically significant(P<0.01).In terms of single score,all scores of the two groups were lower,and the difference was statistically significant as compared with those before treatment(P<0.05).Except for soreness and weakness of waist and knees(P>0.05),the reduction degrees of individual scores in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.01).4.Comparison of modified KUPPERMAN scale scores: The total scores of the treatment group and the control group were decreased after treatment,and the difference was statistically significant(P<0.01),both of which could improve the symptoms of perimenopausal period in patients.As for inter-group comparison,the reduction degree in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).The improvement degree of symptoms in the treatment group was superior to that in the control group.5.Comparison of hormone levels: LH and FSH levels in the treatment group and the control group were lower after treatment than those before treatment(P<0.05),while E2 levels were higher than those before treatment(P < 0.05).After treatment,there was a significant difference in hormone levels between the two groups(P<0.01).6.Comprehensive curative effect on sleep quality: the total effective rate was 91.2% in the treatment group;The total effective rate of that control group was 81.8%.The curative effect of the treatment group was better than that of the control group(P<0.05).7.Follow-up: The total scores of PSQI and TCM syndromes in the two groups were significantly lower than those before treatment and the differences were statistically significant(P<0.01).As for the comparison between the two total scores during follow-up visit,the two total scores of the treatment group were lower than those of the control group,and the difference was statistically significant(P<0.01).The long-term improvement in symptoms by the treatment group was superior to that by the control group.Conclusion:1.Two groups of treatment can improve the degree of insomnia in PMI patients,heart-kidney failure syndrome and perimenopausal symptoms,and both have a long-term effect.Compared with simple oral administration of Kuntai capsule,the efficacy of blood-letting puncture and cupping combined with oral administration of Kuntai capsule is better.2.Blood-letting puncture and cupping therapy can benign regulate HPO axis of perimenopausal women and improve LH,FSH and E2 levels,but further research is required to confirm.3.Blood-letting puncture and cupping combined with Kuntai capsule has significant curative effect on perimenopausal insomnia patients with heart-kidney failing to communicate,which is worthy of clinical promotion. |
