Objective:To compare the differences between PCOS patients with phlegm-dampness syndrome and non-phlegm-dampness syndrome in general clinical characteristics,sex hormone levels,glucose and lipid metabolism indexes and blood uric acid,and analyze the effects of different levels of blood uric acid on the clinical characteristics and endocrine metabolism of PCOS patients with phlegm-dampness syndrome,so as to provide a theoretical basis for the clinical treatment of PCOS patients with hyperuricemia.Methods:From January 2014 to December 2021,a total of 454 patients were selected who were first diagnosed as PCOS by the clinical information integration system of gynecological clinic of Heilongjiang University of traditional Chinese medicine and who measured blood uric acid and entered information in our hospital.Firstly,it was divided into 210 patients with PCOS with phlegm-dampness syndrome and 244 patients with PCOS non-phlegm-dampness syndrome.The general clinical characteristics,sex hormone level,glucose and lipid metabolism and blood UA indexes of the two groups were compared.Then,according to the quartile level of blood UA concentration of 210 patients with PCOS with phlegm-dampness syndrome,they were divided into four groups A,B,C and D.The differences of clinical characteristics,sex hormone level and glucose and lipid metabolism among the groups were compared.Finally,the correlation between blood UA and clinical indexes of PCOS patients with phlegm-dampness syndrome was analyzed.Results:1.The blood UA value of PCOS patients with phlegm-dampness syndrome was significantly higher than that of PCOS patients non-phlegm-dampness syndrome(P<0.05).2.Comparison between group A,group B,group C and group D of PCOS patients with phlegm-dampness syndrome:comparison of general clinical characteristics:there were significant differences in body weight,BMI,WC,HC and WHR between groups with different concentrations of blood UA(P<0.05);The values of body weight,BMI,WC and HC increased with the increase of blood UA level.3.Comparison between group A,group B,group C and group D of PCOS patients with phlegm-dampness syndrome:comparison of sex hormone levels:there was significant difference in T,FAI and SHBG between groups with different concentrations of blood UA(P<0.05);Among them,T and FAI value increases with the increase of blood UA level,and SHBG value decreases with the increase of blood UA level.There was no significant difference in E2level among the four groups(P>0.05),but E2value decreased with the increase of blood UA level.4.Comparison among group A,group B,group C and group D of PCOS patients with phlegm-dampness syndrome:comparison of glucose metabolism indexes:there were significant differences in 60min glucose,FINS,30min insulin,60min insulin,120min insulin,180min insulin,HOMA-IR and HOMA-βbetween groups with different concentrations of blood UA(P<0.05);FINS,60min insulin,180min insulin,HOMA-IR and HOMA-βthe value increased with the increase of blood UA level.5.Comparison among group A,group B,group C and group D of PCOS patients with phlegm-dampness syndrome:comparison of blood lipid metabolism indexes:there were significant differences in TG,HDL-C,Apo-B,Apo-B/Apo-A between groups with different concentrations of blood UA(P<0.05);TG and Apo-B/Apo-A ratio increased with the increase of blood UA level.6.Comparison among group A,group B,group C and group D of PCOS patients with phlegm-dampness syndrome:comparison between the incidence of complications:there were significant differences in IR,Dyslipidemia,Obesity and MS between groups with different concentrations of blood UA(P<0.05);And its incidence increased with the increase of blood UA level.7.Correlation analysis between blood UA and clinical indexes in PCOS patients with phlegm-dampness syndrome:Blood UA level and body weight,BMI,WC,HC,WHR,systolic blood pressure,T,FAI,FPG,60min sugar,120min sugar,FINS,30min insulin,60min insulin,120min insulin,180min insulin,HOMA-IR,HOMA-β,TG,LDL-C,Apo-B,Apo-B/Apo-A were positively correlated(P<0.05);Blood UA level was negatively correlated with E2,SHBG,HDL-C and Apo-A(P<0.05).Conclusion:1.The blood UA level of PCOS patients with phlegm-dampness syndrome was significantly higher than that of PCOS patients non-phlegm-dampness syndrome.2.Patients with PCOS with phlegm-dampness syndrome have high blood UA level,which is correlated with body mass index,glucose and lipid metabolism and hormone level,PCOS patients with phlegm-dampness syndrome under high blood UA level are prone to MS,IR,dyslipidemia and obesity,which can guide the clinical prevention of complications.. |
