With the advent of the aging era,the proportion of the elderly population in the total population is rising,and the resulting social problems are becoming increasingly prominent.Countries all over the world urgently need to take effective measures to solve the pension problem.In recent years,with the rapid growth of China’s aging population,the health problems of the elderly have become more and more prominent.Due to the growth of age and the frequent occurrence of various diseases,some elderly people gradually lose the ability to live a normal life and become disabled or semi disabled elderly people.The elderly group not only needs a variety of elderly care services,but also needs more rich and convenient medical services.The traditional medical institutions and elderly care institutions with single function have been unable to meet the healthy elderly care needs of the elderly.In this context,the combination of medical care and elderly care model came into being,which has become an inevitable choice for China to actively respond to the aging population and meet the healthy elderly care needs of the elderly.Jiangsu Province ranks the third in the country in terms of aging,second only to Beijing and Shanghai,and faces great pension pressure.In 2013,the State Council officially put forward the concept of "combination of medical care and maintenance".Jiangsu Province responded positively and continued to explore the "optimal path" of the combination of medical care and maintenance.By vigorously promoting the health promotion action for the elderly,we will build hospitals for the elderly,rehabilitation hospitals and nursing homes,and strengthen the construction of a service system integrating medical care and maintenance.Jiangsu Province has formed its own characteristic practices and typical experience in the process of practicing the combination of medical care and maintenance.Based on this,this study reviews the development process of medical care combined with elderly care service in Jiangsu Province on the basis of demand tomography theory,welfare pluralism theory and stakeholder theory;Compare and analyze the different characteristics of the typical mode of medical support combination in Jiangsu Province;Using the stakeholder model,this paper analyzes all kinds of participants(core stakeholders,intermediate stakeholders and marginal stakeholders)in the combination of medical care and elderly care model,and explains the interest demands of different subjects and the practical difficulties they face.The study found that there are the following problems in the practice of the combination of medical care and elderly care model in Jiangsu Province: decisive stakeholders face the dilemma of disconnection between supply and demand;Expectant stakeholders face the dilemma of management and coordination;Potential stakeholders face the dilemma of diversified supervision.Therefore,optimization strategies are proposed for different stakeholders: service providers need to constantly improve the evaluation and grading mechanism,speed up the training of medical and nursing professionals,and provide diversified medical and nursing services;Service demanders need to strengthen their understanding of the combination of medical care and elderly care,actively participate in the service of the combination of medical care and elderly care,and give timely feedback and suggestions;Managers need to improve relevant laws and regulations on the combination of medical care and nursing,increase investment,protect the rights and interests of medical staff,and attract extensive participation of social capital;The coordinator needs to increase the integration of community service platforms,improve community medical and elderly care service facilities,and improve the content of community medical and elderly care services;Regulators need to publicize and advocate the mode of combining medical care with elderly care,build a smooth overall coordination and linkage system,and establish a practical and effective supervision and evaluation system.
