The Effect Of Perioperative Management Scheme Based On Mind Map In Preoperative Expectation Of Cataract Patients

Objective:Through the investigation to understand the current situation of preoperative visual function expectation of cataract patients and analyze its influencing factors,the semi-structured interview method and the expert meeting method were used to construct a mind map-based perioperative management scheme for cataract patients and evaluate its application effect.Methods:(1)A total of 146 cataract patients’ preoperative visual function expectation admitted to a tertiary eye hospital from January 2021 to May 2021 were investigated,and their influencing factors were analyzed.(2)Semi-structured interviews were conducted with 12 cataract patients in a tertiary eye hospital to understand the needs of patients’ perioperative management.(3)On the basis of consulting and analyzing relevant domestic and foreign literature,referring to the expert consensus and guidelines,combined with the current status investigation and semi-structured interviews in the early stage,and the mind map-based draft of perioperative management of cataract patients was initially formed.Through the expert meeting method,the draft of perioperative management was further revised and improved,and the final version of the perioperative management scheme for cataract patients was formed and the mind maps were designed with X-Mind 8.0software.(4)A total of 122 cataract patients in a tertiary eye hospital were selected,the subjects admitted from June to August 2021 were included in the control group,and the subjects admitted from September to November 2021 were included in the intervention group.The control group were used the traditional perioperative management scheme,and the intervention group were used the mind map-based perioperative management scheme.The general information questionnaire,the Chinese version of the visual function index scale(VF-12-CN),the health knowledge questionnaire,and the nursing satisfaction questionnaire were used as survey tools to investigate the cataract patients before and after the intervention.compare the differences in patient general demographic characteristics,preoperative visual function expectations,the difference between preoperative visual function expectation and postoperative visual function,visual satisfaction,knowledge about health care and nursing satisfaction.Results:(1)A total of 146 patients were included in the cross-sectional study.The results showed that the preoperative visual expectation of cataract patients was 97.73(91.67,100);while the actual postoperative visual function was 89.58(83.33,93.75)points,the difference between preoperative visual function and postoperative actual results needs to be improved.The multivariate stepwise regression analysis showed that the main factors affecting the preoperative expectation of cataract patients were the history of cataract surgery,educational level,and Eysenck personality E.(2)The contents of cataract perioperative management needs derived from semi-structured interviews with 12 cataract patients include: knowledge about cataract disease;types and functions of intraocular lenses;factors affecting postoperative visual function recovery;perioperative diet,medication,post-operative precautions,regular review and other self-care knowledge,as well as possible complications prevention and countermeasures;surgical procedures,anesthesia methods,and intraoperative cooperation,etc.(3)Through literature analysis,current situation investigation and qualitative interview results,with reference to the consensus and guidelines of domestic and foreign experts,the mind map structure framework and first draft of the perioperative management scheme were constructed.Eight experts including ophthalmology,nursing management and ophthalmic nursing discussed and revised the preliminary perioperative management scheme,formed the final version of the perioperative management scheme for cataract patients,and used X-Mind 8.0 software to design the scheme into mind maps.(4)The similar experimental study included 122 subjects,including 61 in the intervention group and 61 in the control group.There was no significant difference in baseline between the two groups(P>0.05).The preoperative visual function expectancy scores of the patients before and after the intervention in the group were compared,and the results showed that the preoperative expectancy scores of the patients in the intervention group after the intervention were significantly lower than those before the intervention(Z=-6.017,P<0.05);The preoperative visual function expectancy scores and the difference between preoperative visual function expectation and postoperative visual function were compared between the two groups,respectively,and the results showed that the preoperative expectation score of the intervention group was 85.42(11.36)points,which was lower than the control group’s 97.2(6.25)points,and the difference was statistically significant(Z=-5.608,P<0.001);the difference between preoperative expectation and postoperative visual function in the intervention group was 0(7.67)points,while that in the control group was 8.33(8.33)points,and the difference was statistically significant(Z=-4.248,P<0.001).The visual satisfaction,nursing satisfaction and knowledge about health care of the intervention group were higher than those of the control group,and the difference was statistically significant(Z=-2.997,P=0.003;t=14.336,P<0.001;t=4.535,P<0.001).Conclusion:(1)The preoperative visual function of cataract patients are at a high level,and there is a large difference between the preoperative visual function and the actual results,indicating that the preoperative visual function expectations of cataract patients do not conform to the actual postoperative visual function.Identifying patients’ unreasonable expectations and adjusting them early is an urgent problem for ophthalmic medical staff.The factors that affect the patients’ preoperative visual function mainly include the history of cataract surgery,educational level,Eysenck personality E,etc.In the later stage,it is necessary to formulate an intervention scheme with reference to influencing factors to guide patients to make more reasonable preoperative visual function.(2)Through literature analysis,cross-sectional survey,qualitative research,and expert meeting,the perioperative management scheme for cataract patients based on mind map was constructed.The scheme can effectively adjust the unreasonable preoperative visual function of cataract patients,make preoperative visual function more in line with postoperative visual function,and improve patients’ visual satisfaction,nursing satisfaction and knowledge about health care.
