| Objectives1.To develop standardized discharge guidance for patients undergoing coronary stent implantation based on Delphi expert consultation method,a mind map was constructed.2.To evaluate the effect of mind mapping based discharge guidance on the discharge readiness of patients undergoing first-time coronary stenting,this study provided a reference for clinical nursing practice of improving the self-management of patients after discharge and reducing the readmission rate.3.To investigate the effect of mind mapping based discharge guidance on the quality of discharge guidance for patients undergoing first coronary stenting.4.To explore the effective method of discharge guidance for patients undergoing coronary stent implantation.MethodsThis project is divided into two parts.The first part,on the basis of literature review and group discussion,uses Delphi expert consultation method to construct the content of discharge guidelines for patients with coronary stent implantation,and draws a mind map.The second part is to apply mind mapping to the discharge guidance of patients undergoing first coronary stenting to verify the effect of this method on the discharge readiness of patients.1.Through literature review and group discussion,the framework indicators of discharge guidance for patients undergoing coronary stent implantation were preliminarily established.In nine districts of Delphi expert consultation method was applied to 11 third rate is a comprehensive teaching hospital 24 engaged in cardiovascular research two rounds of consulting experts,according to the expert advice in patients with coronary artery stent implantation discharge guidance index modification and improvement of the framework of the application of SPSS 25.0software for statistical data analysis,finally make a coronary stent implantation patients discharged from hospital to guide the main content,draw a mind map.2.Using continuous sampling method,the selection between December 2019 and August 2020 a third rate in Dalian city,the hospital heart 100 cases in hospital with coronary stent implantation patients as the research object,for the first time in December 2019 to May 2020 in the hospital of 50 cases in control group,in June 2020to August 50 patients into the experimental group in the hospital.The control group was given routine discharge guidance,while the experimental group was given discharge guidance with mind map.The research tools were used to compare the differences between the two groups in terms of patient general data questionnaire,discharge readiness scale and discharge guidance quality scale.In this study,SPSS 25.0 software was used for data processing and analysis,and the statistical methods used mainly included descriptive statistical analysis,c~2test and two independent sample T-test.Results1.In this study,the effective recovery rates of the two rounds of expert consultation questionnaires were 91.67%and 100%respectively,and the expert authority coefficients were higher than 80%.In the first round of expert consultation,Kendall coordination coefficient W was 0.117,0.165 and 0.184 respectively(P<0.05).The average value of importance of all levels of indicators was higher than 3.73,with a full score rate of 18.18%to 100%and a coefficient of variation of 0 to 30.01%.In the second round of expert consultation,Kendall coordination coefficient W was 0.131,0.298,and 0.196(P<0.05),respectively.The average value of importance assigned to all levels was higher than 4.18,with a full score rate of 36.36%to 100%,and a coefficient of variation of 0 to 21.85%.Experts gradually reached consensus.2.The final discharge guidance in this study includes 7 first-level indicators(diet,exercise,drugs,control of risk factors,knowledge related to stent complications,home first aid,and follow-up),28 second-level indicators,and 57 third-level indicators.3.The comparison of general data between the two groups showed no statistically significant difference(P>0.05),indicating comparability.4.The total score of discharge readiness,personal status,adaptive ability and expected support in the experimental group were(107.04±8.92),(25.18±3.79),(45.24±3.63)and(36.62±3.63),all higher than those in the control group,and the differences were statistically significant(P<0.05).5.The total score of discharge guidance quality of patients in the experimental group was(151.12±8.26)points,the actual obtained content was(56.66±6.06)points,the guiding skill and effect was(94.46±4.33)points,and the dimension of content difference was(12.22±6.07)points,all of which were higher than those in the control group and the differences were statistically significant(P<0.05).There was no significant difference in the content dimension scores between the two groups(P>0.05).Conclusions1.The discharge guidance for patients undergoing coronary stent implantation based on Delphi expert consultation method is scientific and authoritative.2.The application of mind mapping based discharge guidance method can improve the discharge readiness of patients undergoing first coronary stenting.3.The implementation of mind mapping based discharge guidance method is helpful to improve the quality of discharge guidance for patients undergoing first coronary stent implantation.4.Mind mapping can be used as an effective method to guide patients discharged from hospital after coronary stent implantation and is worth popularizing and applying in clinical nursing practice. |
