Objective:Gallbladder and biliary diseases is a common gastrointestinal disease with the huge socioeconomic costs and has become an common cause for hospital admissions among gastrointestinal conditions.The lifestyle and dietary habit of people have changed,the prevalence of metabolic syndrome,type 2 diabetes mellitus,obesity and ageing trend have increased in recent years,the epidemic situation of gallbladder and biliary diseases has changed as those and the burden and medical cost due to diseases have also sharply increased.There are relevant studies on gallbladder and biliary diseases burden which limited to several regions and countries.This study aimed to systematically evaluate the burden of gallbladder and biliary diseases in global and regions based on GBD database,to provide evidence for preventing and controling the gallbladder and biliary diseases.Methods:In the study,we used the number and age-standardized rate of incidence,prevalence,years of life lost(YLLs),years lived with disability(YLDs),deaths for gallbladder and biliary diseases and related disease burden attributable to risk factors in global and different regions from Global Burden of Disease Study 2019(GBD2019),to analyze the trends and the related sex and age differences in the gallbladder and biliary tract disease burden globally and in different regions.We obtained the current health expenditure per capita in US$(CHE_pc_US$)across 187 countries in 2000-2019 from Global Health Expenditure Database,to assess the relation between the gallbladder and biliary tract disease burden to the relevant health expenditure,and the trend in current health expenditure across different countries.Results:1.the burden of gallbladder and biliary tract disease in global and different regions from 2000 to 2019Globally,the number of prevalence,incidence,YLLs,YLDs,deaths and the age-standardized incidence rate for gallbladder and biliary diseases have increasd during the study.The age-standardized prevalence,YLLs,YLDs,rate have declined in the past 20 years.The number of prevalence,incidence,YLLs,YLDs,deaths increased to 193493.38 thousand(166626.34-229375.43),52003.77 thousand(44202.14-61211.62),2290.90 thousand(1970.33-2524.61),4061.84 thousand(2595.49-5953.10),124.94 thousand(107.11-138.61),and the age-standardized rate of prevalence,incidence,death,YLL,YLD were 2350.78 per 100 000(2029.59-2778.69),634.32 per 100 000(540.21-742.93),1.65 per 100 000(1.41-1.84),28.92 per 100 000(24.89-31.76),49.33 per 100 000(31.51-71.96)in2019,respectively.The age-standardized rate of prevalence,incidence,YLDs increased as the income level across different regions,and the highest age-standardized rate of death and YLL appeared at low income region during the past 20 years.The number of prevalence,incidence,YLDs,YLLs,deaths,age-standardized incidence rate for gallbladder and biliary diseases have increased across high to low income regions during the study time.The age-standardized rate of YLLs and deaths have decreased across different income regions,and the age-standardized rate of prevalence and YLDs decreased across high income,upper middle income and low income regions in the same time.The age-standardized incidence,prevalence and YLDs rate for female were greater than male across different income regions.People aged 75 years and above have the highest rate of prevalence,incidence,YLLs,YLDs,deaths for gallbladder and biliary disease,the higherly rise of incidence rate among 25-49 age group was found in global and high to low income regions during the study time.the decreased trend of YLDs and prevalence rate among this group was slow in global and high to low middle income regions in the same time.2.The gallbladder and biliary disease burden due to risk factors in global and different regions from 2000 to 2019Smoking and high body mass index(BMI)were risk factors contribute to gallbladder and biliary disease,high BMI was the main risk factor for gallbladder and biliary disease.The disease burden due to high BMI have increased from 2000 to2019 wordwide.Globally,the number of YLDs,YLLs,death due to high BMI were1337.89 thousand(692.11-2339.11),709.55 thousand(442.97-1025.44),39.13thousand(24.00-57.57),and the age-standardized rate of YLLs,YLDs,death due to hih BMI were 8.84 per 100 000(5.51-12.79),16.10 per 100 000(8.34-28.28),0.51 per 100 000(0.31-0.76)in 2019,respectively.The age-standardized YLD rate due to high BMI have increased in upper middle to low income regions,and the fastest rising trend were found in low middle to low income regions.The age-standardized rate of YLLs and death due to high BMI have rapidly increased at low middle and low income regions.Moreover,the fastest rise of the YLD rate due to high BMI was in 25-49 age group among people aged 25 and above years in global and all income regions.Conclusion:The gallbladder and biliary diseases burden were not sufficiently controlled and the incidence rate has increased in global during the past 20 years.Senior and females were the high-risk groups for gallbladder and biliary diseases.A young trend in incidence for gallbladder and biliary diseases was found in global and high to low income regions,and the gallbladder and biliary diseases burden across young people was poorly controlled.The diseases burden due to high BMI have increased in low middle,low income regions.Notablely,the sharp increase in the YLD rate due to high BMI was found among young adults in global and all income regions.
