| Objective:ERAS concept combined with percutaneous acupoint stimulation applied to femoral neck fracture after total hip replacement,observation of early postoperative patients with lower limb venous thrombosis occurs,clear percutaneous acupoint stimulation prevent efficacy and safety of the lower extremity venous thrombosis,femoral neck fracture surgery for clinical provide new ways to prevent venous thrombosis of lower limbs.Method:According to the inclusion and exclusion criteria,84 patients who received total hip arthroplasty for femoral neck fracture in Fuzhou Second Hospital affiliated to Xiamen University from January 1,2021 to January 31,2022 were selected.All patients were randomly divided into control group and experimental group,with 42 cases in each group.ERAS concept was used to prevent thrombosis during perioperative period in both groups,including pre-rehabilitation management,pain management,nutrition management and blood management,On the basis of the treatment of the control group,the experimental group began to use percutaneous acupoint electrical stimulation intervention on the 1st postoperative day.The incidence of postoperative venous thrombosis in lower limbs was compared between the two groups,and collected the changes of d-dimer,FIB,velocity of blood flow and diameter of common femoral vein before and after treatment,SPSS25.0 statistical software was used to analyze the data and compare the clinical efficacy between the two groups.Result:A total of 81 patients were followed up completely in this study,including 41 in experimental group and 40 in control group.1.General data: There was no statistical difference between the two groups in gender,age,body mass index,surgical site,Preoperative preparation time,surgical duration,intraoperative blood transfusion and other aspects,indicating comparability.2.Comparison of FIB concentration: there was no difference in FIB concentration between the two groups before treatment(P>0.05);After 1 week of treatment,FIB concentration in experimental group was significantly lower than that in control group,and the difference was statistically significant(P<0.05).3.Comparison of D-dimer concentration: there was no significant difference in D-dimer concentration between the two groups before treatment(P>0.05);After 1 week of treatment,D-dimer concentration in experimental group was significantly lower than that in control group,and the difference was statistically significant(P<0.05).4.Vascular diameter of femoral vein: there was no statistical significance in vascular diameter of femoral vein between the two groups before treatment(P>0.05);After 1 week of treatment,the inner diameter of the femoral common vein was significantly widened in both groups,but the difference was not statistically significant(P>0.05).5.The velocity of the total femoral vein: there was no statistically significant difference in femoral vein blood flow velocity between the two groups before treatment(P>0.05);After 1 week of treatment,the blood flow velocity of common femoral vein in experimental group was significantly faster than that in control group,and the difference was statistically significant(P< 0.05).6.Occurrence of venous thrombosis of lower limbs: No thrombosis of lower limbs was found in both groups before surgery and before treatment.One week after surgery,4patients in the control group and 1 patients in the experimental group were found to have lower limb venous thrombosis.The incidence of lower limb venous thrombosis in the control group was significantly higher than that in the experimental group,but the difference was not statistically significant(P>0.05).conclusion:ERAS concept combined with percutaneous acupoint electrical stimulation can improve blood hypercoagulability after femoral neck fracture,improve venous blood flow velocity of lower limbs,and effectively reduce the incidence of venous thrombosis of lower limbs,with high safety and positive significance in clinical application. |
