| Purpose:this study aims to observe huoxue soup unclogged arteries in combination with small dose of low molecular heparin sodium and normal dose low molecular heparin sodium total hip replacement in patients with femoral neck fracture(total hip arthroplasty,THA)after lower extremity deep vein thrombosis(deep venous thrombosis,DVT)to prevent the clinical curative effect of contrast,to explore the mechanism of action of huoxue soup unclogged arteries and the unique advantages of Chinese medicine,for better use of traditional Chinese medicine to prevent THA postoperative lower limb DVT and provide reference for related complication.Methods:According to the principle of random grouped,60 patients with femoral neck fracture who were to receive THA were divided into two groups:experimental group and control group.The control group was treated with normal dose of low molecular weight heparin sodium(Xifuquan,0.4ml:4250IU),and the experimental group was treated with Huoxue Tongmai Decoction combined with small dose of low molecular weight heparin sodium(Xifuquan,0.2ml:2125IU).After 7 days of continuous use,blood coagulation indexes,lower limb DVT incidence,postoperative VAS score,hemoglobin(Hb),postoperative thigh diameter difference and related adverse reactions were collected and analyzed.Results:1.The general clinical data of patients in the two groups,including gender,age,hip site,injury time from surgery,showed no statistical significance(P>0.05),and the data were comparable.2.There were significant differences in APTT and PT values between the two groups in intra-group comparison,preoperative and postoperative indicators(P<0.05),postoperative APTT and PT values of the two groups could be improved(P<0.05),but there was no difference between the two groups(P>0.05).In the comparison of D-dimer between the two groups,there were significant differences between preoperative and postoperative indexes between the two groups(P<0.05),both groups could reduce the postoperative D-dimer value(P<0.05),but there was no difference between the two groups(P>0.05).There was no statistical significance in the incidence of DVT in lower limbs between the two groups(P>0.05).3.In terms of the comparison of VAS score and Hb between the two groups,both postoperative Hb value and postoperative VAS score could be improved by intra-group comparison(P<0.05).The effect of experimental group on Hb and VAS score on postoperative day 3 and postoperative day 7 was significantly better than that of the control group(P<0.05).4.As for the comparison of the perioperative diameter difference of the thighs between the two groups,intra-group comparison showed that the diameter difference of the thighs on the 7th day after surgery was decreased in both groups compared with that on the 1st day after surgery.Intergroup comparison showed that the diameter difference of the thighs in the experimental group was significantly lower than that in the control group on the 7th day after surgery(P<0.05).5.In terms of postoperative adverse reactions,the number of subcutaneous ecchymosis in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:huoxue soup unclogged arteries in combination with small dose of low molecular heparin sodium to prevent femoral neck fracture THA postoperative lower limb DVT has good effect,huoxue soup unclogged arteries have good anticoagulant effect,and used alone,compared to conventional dose low molecular heparin sodium huoxue soup unclogged arteries also has alleviate patients postoperative swelling pain,promote the recovery of Hb in patients with unique advantages,but also reduced the occurrence of adverse reactions,There was no statistical significance in wound healing(P>0.05). |
