| Background: The main purpose of this study was to investigate the clinical effect of ERAS combined with traditional Chinese medicine on the rehabilitation of total hip arthroplasty in elderly patients with femoral neck fracture.By observing the patient’s hip activity score,pain score and ground time,postoperative stress response,etc.The difference between the situation and the control group was objectively evaluated.The clinical application effect of ERAS combined with traditional Chinese medicine on the rehabilitation of the elderly patients with femoral neck fracture after total hip arthroplasty was provided.The clinical basis for the further promotion and application of the therapy in this study was provided.Methods: According to the study design,60 patients with age-related femoral neck fractures who met the requirements of the Department of Orthopaedics of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from December 2016 to December 2018 were selected,and 60 patients who met the criteria and were required to undergo surgery were selected.For the same group of senior doctors,the total hip arthroplasty wasused,and the prosthesis was selected as a bioprosthesis.According to the patient’s admission date,the observed cases were divided into two groups according to the random number table method,30 cases in the treatment group and 30 cases in the control group.Using the cohort-controlled study method,the treatment group was given ERAS+ Chinese medicine orally in the perioperative period,and the control group was given a general treatment plan using the ERAS concept in the perioperative period.After the treatment,the postoperative VAS pain scores were compared between the two groups.The post-hip hip Harris score,postoperative ground time,postoperative hospital stay,hospitalization cost,and postoperative C-reactive protein and insulin resistance index values were used to determine clinical efficacy.Results: Comparison between the two groups:(1)Haris scores at 1 week and 2weeks after hip surgery(P<0.05).(2)Comparison of Hariss scores between 1month and 2 months after hip surgery(P>0.05).(3)The VAS scores of the two groups were compared between the two groups.The comparison of the VAS scores between the two groups(P>0.05)was not statistically significant.The comparison of VAS scores at 7 and 14 days after operation was P<0.05.The difference was statistically significant;intra-group comparison: 7 days and 14 days after surgery compared with 3 days after surgery(P<0.05),the difference was statistically significant.(4)The postoperative time was shortened(P<0.05),postoperative hospital stay was reduced(P<0.05),and hospitalization expenses were reduced(P<0.05),which was statistically significant.(5)Thecomparison of postoperative C-reactive protein between the two groups,1 day,3 days,5 days after surgery,P <0.05,statistically significant.(6)The comparison of postoperative insulin resistance between the two groups,1 day,3 days,5 days after surgery,P <0.05,statistically significant.(7)The two groups of patients before and after treatment TCM syndrome scores,TCM syndrome efficacy comparison,the treatment group was better than the control group,indicating that Chinese medicine oral administration can improve the symptoms of postoperative qi and blood deficiency,and promote postoperative rehabilitation.Conclusions:(1)The results of this study indicate that ERAS combined with traditional Chinese medicine can effectively improve the recent Harris score in patients with femoral neck fracture in the elderly and promote rapid postoperative recovery.(2)Based on ERAS combined with traditional Chinese medicine orally,the treatment of total hip arthroplasty in elderly patients with femoral neck fracture can effectively relieve postoperative pain,improve the general condition of the patient,promote postoperative recovery,reduce postoperative stress response,reduce insulin resistance,and shorten Downtime,reduce patient stays and reduce hospitalization costs.(3)The treatment plan based on ERAS combined with traditional Chinese medicine can promote the effect of accelerated rehabilitation surgery on postoperative rehabilitation,and it is worth further promotion in the clinic. |
