Clinical Observation On The Treatment Of Granulomatous Mastitis In The Late Stage Of Ulceration With The Triple External Treatment Of "Search、Flush、Pressure"

Objective:To study the clinical effect of "Search、flush、pressure" triple external treatment on the late stage of granulomatous mastitis,in order to provide basis for treatment.Methods:Patients with granulomatous mastitis in the late stage of ulceration treated in the breast and thyroid surgery clinic or ward of the Affiliated Hospital of Shandong University of traditional Chinese medicine from October 2020 to October 2021 were selected.A total of 60 patients were selected according to the inclusion and exclusion criteria,with 30 patients in each group.The treatment group used the triple external treatment method of "search 、impact、pressure",and the control group underwent abscess debridement,Both groups were treated with Tuoli disinfection powder and Bazhen decoction.The clinical efficacy and safety of the two groups were comprehensively evaluated by comparing the reduction rate of wound area,the improvement rate of traditional Chinese medicine symptoms,breast shape and other indicators of the two groups at week 2,week 4,week 6 and week 8.The patients were followed up for 3 months after treatment,and the recurrence rate was counted.All data were passed through spss26.0 software to observe the differences between the two groups.Result:(1)After treatment,the wound area of the two groups decreased.Statistical analysis showed that there was no significant difference between the two groups at the second and fourth weeks(P > 0.05).The treatment group was better than the control group at the sixth and eighth weeks(P < 0.05).After treatment,the reduction rate of wound area in the treatment group(51.47%)was higher than that in the control group(28.47%),and the difference was statistically significant(P < 0.05);(2)In terms of breast shape,economic indexes and patients’ self satisfaction,the treatment group was significantly better than the control group(P < 0.05);(3)The TCM symptom scores of patients decreased,and their symptoms such as anxiety,insomnia and fatigue were improved.There was no significant difference between the two groups at the second and fourth weeks(P > 0.05).The effect of the treatment group was better than that of the control group at the sixth and eighth weeks(P <0.05).Compared with the improvement rate of TCM symptoms,the treatment group was better than the control group(P < 0.05);(4)There was no significant difference in VAS score between the two groups at each observation node(P > 0.05);(5)The total effective rate in the treatment group was higher than that in the control group(96.67% vs 70%),and the difference was statistically significant(P < 0.05);(6)The recurrence rate in the control group was higher than that in the treatment group(9.52% vs 3.44%)(P < 0.05);(7)During the experimental study,the safety evaluation and adverse reactions of the two groups were normal.Conclusion:The triple external treatment of "search 、 flush 、 pressure" and abscess debridement are effective in the late stage of granulomatous mastitis.Compared with abscess debridement,the triple external treatment of "search 、 flush 、 pressure" has significant advantages of reducing wound area,protecting breast shape,reducing economic pressure and high patient self satisfaction.The clinical operation is relatively simple and feasible.
