Clinical Observation Of Yishen Jianzhong Decoction In Treating Diabetic Kidney Disease Of Spleen And Kidney Qi Deficiency And Blood Stasis Type

Objective :In this study,we analyzed the clinical data to observe the clinical efficacy and safety of Yishen Jianzhong Decoction in the treatment of stage III、IV Diabetic Kidney Disease with spleen and kidney qi deficiency and blood stasis,providing new ideas and new methods for the diagnosis and treatment of Diabetic Kidney Disease with spleen and kidney qi deficiency and blood stasis in traditional Chinese medicine.Methods :Sixty-four patients who met the inclusion criteria were divided into control group(n = 32)and observation group(n = 32)according to random number table.The two groups were given basic treatment and conventional western medicine treatment,the control group was given benazepril on this basis,the observation group was given Yishen Jianzhong Decoction on the basis of the control group,the two groups were continuously treated for 2 courses,a course of treatment was 4 weeks.The TCM score,TCM efficacy,glucose metabolism(fasting plasma glucose,2-hour plasma glucose,glycosylated hemoglobin),lipid metabolism(triglyceride,low-density lipoprotein),serum cystatin C,urinary microalbumin,24-hour urinary protein quantification and safety indicators(vital signs monitoring,blood routine,urine routine,stool routine,liver function,electrolytes)and adverse reactions were observed before and after treatment in both groups,and the obtained data were statistically analyzed by SPSS 25.0 software.Results:⑴There was no significant difference in age,gender and disease course between the two groups(P>0.05),indicating comparability.⑵There was no significant difference in glucose metabolism(FPG,2h PG,Hb A1c),lipid metabolism(TG,LDL-C),Cys C,m ALb,24 h UPQ,total score of TCM and each single score before treatment between the two groups,P>0.05,indicating comparability.⑶The FPG,2h PG,Hb A1 c,TG,LDL-C,Cys C,m ALb,24 h UPQ and total score of TCM before and after treatment in the two groups were significantly different(P<0.05);the difference between the two groups after treatment was statistically significant(P<0.05).⑷In terms of each single TCM score,there was a statistically significant difference in each symptom P<0.05 within the two groups.After treatment,the differences between the two groups had statistical significance(P<0.05)in thirst gradually did not want to drink,frequent urination at night,poor appetite,fatigue,soreness of waist and knee and loose stool;the differences in turbid urination,limb numbness or pain,edema and cyanotic lip nails had no statistical significance(P>0.05).⑸In terms of TCM efficacy evaluation,after treatment,the overall response rate of the observation group was 93.3%,the control group response rate was70%,P<0.05,and the difference was statistically significant.⑹In terms of safety and adverse reactions,the vital signs of patients in the two groups were stable,all safety observation indicators were not significantly abnormal compared with those before treatment,and no adverse reactions occurred.Conclusion:Compared with the benazepril alone group,the Yishen Jianzhong Decoction combined with benazepril group is more effective in the treatment of stage III、IV Diabetic Kidney Disease with spleen and kidney qi deficiency and blood stasis,which can effectively improve various clinical symptoms,reduce blood glucose,regulate blood lipids,protect renal function,and reduce urinary protein,and has good safety.
