| Objective: In this study,the clinical efficacy of "Yi Shen Gu Ben Decoction" in the treatment of stage 3 of chronic kidney disease(CKD)with spleen and kidney deficiency and wet turbid blood stasis type was observed.We analyze its influence on clinical laboratory indicators and changes of clinical syndrome integral,and objectively and scientifically evaluate its clinical efficacy and safety,for providing theoretical basis for TCM treatment of chronic kidney disease.Methods: All cases observed were diagnosed as a stage 3 of chronic kidney disease in the outpatient department and ward of the first Department of Nephrology of Heilongjiang Academy of Traditional Chinese Medicine during may 2017 to march 2019.They met the inclusion criteria,totaling 64 cases,3 cases of abscission and 61 cases of final effective cases.The treatment group(31 cases)and the control group(30 cases).Treatment group:Routine treatment combined with Yishen Guben Decoction was given orally twice a day for 150 ml.Control group: Routine treatment combined with Niaoduqing Granule(Guangzhou Kangchen Pharmaceutical Co.,Ltd.)taking orally three times a day for 10 g and the course of treatment is 8 weeks.The creatinine(Scr),glomerular filtration rate(eGFR),urea nitrogen(BUN),24 h urine protein quantitation,hemoglobin(Hb),Fibrinogen(FIB),liver function(ALT、AST),blood fat(TC、TG),TCM syndrome scores,and efficacy evaluation were observed in the two groups.Data analysis was performed using SPSS 20.0 statistical software.Results:(1)Total efficacy evaluation: The effective rate of the treatment group was 90.3%,which was better than 76.7% of the control group,(P < 0.05)with statistical significance.(2)Evaluating the curative effect of TCM syndromes: the effective rate of the treatment group was 90.3%,which was better than 73.3% of the control group,(P < 0.05)with statistical significance;(3)TCM syndromes total score: after treatment,the two groups showed a significant downward trend,the treatment group was significantly better thanthe control group,(P < 0.05)with statistical significance;(4)TCM clinical symptoms: the treatment group was improving fatigue,low back ache and soreness and weakness of waist and knees,shortness of breath.The treatment group was significantly better than the control group,(P < 0.05)with statistical significance.(5)Renal function: creatinine and urea nitrogen(BUN)in the two groups decreased significantly after treatment,while the treatment group was better than the control group,(P < 0.05)with statistical significance.Glomerular filtration rate in the two groups increased significantly after treatment.The rising trend of the treatment group was better than that of the control group,(P < 0.05)with statistical significance;(6)24h urine protein quantitation: two groups after treatment,24 h urine protein quantitation in the treatment group was lower than that in the control group,(P < 0.05)with statistical significance.(7)Fibrinogen(FIB): two groups after treatment,there was a decrease compared with before,and the decline trend in the treatment group was better than that in the control group,(P < 0.05)with statistical significance.(8)Blood lipids(TG,TC): two groups after treatment,there was a decrease compared with before,and the decline trend in the treatment group was better than that in the control group,(P <0.05)with statistical significance.(9)Hemoglobin: There was no significant difference between the two groups after treatment,(P > 0.05)with no statistical significance.(10)Safety indicators: liver function(ALT,AST)after treatment than before in two groups,no significant change,(p > 0.05)with no significant difference and no statistical significance.Conclusion: 1 Yishen Guben Decoction can significantly improve the clinical symptoms of stage 3 of chronic kidney disease(CKD)with spleen and kidney deficiency and wet turbid blood stasis type,such as deficiency of spleen and kidney Qi,fatigue,soreness and weakness of waist and knees,shortness of breath,laziness in eating,dull complexion,wrong skin armor and bitter mouth,and improve the quality of life of patients.2 Yishen Guben Decoction can reduce the levels of serumScr and BUN in stage 3 of chronic kidney disease(CKD)patients with spleen and kidney deficiency and wet turbid blood stasis type,and improve glomerular filtration rate and delay the progress of CKD.3 Yishen Guben Decoction can reduce 24 h urine protein quantitation of stage 3 of chronic kidney disease(CKD)patients with spleen and kidney deficiency and wet turbid blood stasis type,and protect glomeruli and delay the progress of CKD.4 Yishen Guben Decoction can improve the disorder of lipid metabolism,decrease fibrinogen,improve blood viscosity and kidney blood flow of stage 3 of chronic kidney disease(CKD)with spleen and kidney deficiency and wet turbid blood stasis type. |
