External Washing Of Traditional Chinese Medicine For Palmoplantar Pustulosis:Rules Of Herbal Administration And Meta Analysis

Objective Through searching and collating Chinese and foreign literature on the treatment of palmoplantar pustulosis(PPP)with external washing of traditional Chinese medicine in recent years to provide reference and ideas for the treatment,and researching and developing the Chinese medicine preparation for external use to treat PPP.A systematic review of the efficacy and safety of external washing of traditional Chinese medicine treatment of PPP and to obtain more reliable evidence-based medical evidence for clinical treatment.Method Study on the rules of herbal administration of external washing of traditional Chinese medicine treatment of PPP:By searching databases,collecting literature,extracting relevant data after screening to establish a database of literature and drug composition,a unified database of drug names,and a database of information on external washing of traditional Chinese medicine,and using descriptive statistics and hierarchical clustering analysis for statistical analysis.Meta-analysis of external washing of traditional Chinese medicine treatment for PPP:By searching and collating Chinese and foreign literature on external washing of traditional Chinese medicine treatment for PPP in recent years,Meta-analy s i s was conducted after screening the included literature to evaluate the efficacy and safety of external washing of traditional Chinese medicine treatment of PPP.Results Study on the rules of herbal administration of external washing of traditional Chinese medicine treatment of PPP:A total of 13 related papers were included,involving 40 herbs with a cumulative frequency of medication administration was 98 times,among which the frequency of use of a single herb is greater than or equal to 4 times were Ku Shen,Huang Bo,Bai Xianpi,Di Fuzi,Bai Bu,Gan Cao,Ma Chixian,Pu Gongying,and She Chuangz i.After systematic cluster analysis of these 9 high-frequency core herbs,2 classifications were obtained.Classification 1 contained Huang Bo,Ku Shen,Bai Xianpi,and Pu Gongy ing,all of which have the effects of clearing heat,drying dampness,and detoxification;Classification 2 contained She Chuangzi,Di Fuzi,Ma Chixian,Bai Bu and Gan Cao,all of which have the effects of dispelling wind,clearing heat,removing dampness,killing insects,relieving itch and diuresis.Meta-analysis of external washing of traditional Chinese medicine treatment for PPP:A total of 10 relevant papers were included,and the treatment involved 730 patients,including 368 cases in the experimental group and 362 cases in the control group.The lit erature was tested by funnel plot without significant publication bias.Metaanalysis shows that there was a statistically significant difference in the efficacy of the two treatment regimens for PPP[OR=5.36,95%CI(3.54,8.13),P<0.01],while the difference in the adverse event incidence rate between the two regimens for PPP was not statistically significant.Conclus ion In the prescription of external washing of traditional Chinese medicine,Huang Bo,Ku Shen,Bai Xianpi are selected as the set of core herbs to play the therapeutic effect of clear heat and dry dampness,and the auxiliary herbs such as She Chuangzi,Di Fuzi,Ma Chixian and Bai Bu are selected to play the therapeutic effect of di spelling wind,removing dampness,killing insects and relieving itch.On the basis of conventional treatment,external washing of traditional Chinese medicine for PPP can improve the efficacy of treatment without increasing the incidence of adverse events.
