| Objective:1.The purpose of the present study was to retrieve systematically and collate the ancient and modern literature related to the treatment of chronic prostatitis with traditional Chinese medicine external treatment,to mine its clinical medication rules and provide a theoretical basis for the study of chronic prostatitis external treatment.2.The purpose of the present study was to evaluate systematically the clinical efficacy and safety of Chinese medicine external treatment for chronic prostatitis.Methods:1.Study on the rules of clinical medication.Using China Knowledge Network Database(1979-2018),Wanfang Database(1989-2018)and Chinese Science and Technology Database Weipu Information(1989-2018)as the source of modern literature retrieval,and Chinese medical classics as the source of ancient literature retrieval,to retrieve the literature on chronic prostatitis treated with traditional Chinese medicine.The statistical software SPSS21.0 was used for statistics,induction and analysis,and the clinical medication rules of traditional Chinese medicine treatment for chronic prostatitis were summarized.2.Systematic review:Using comprehensive search of China Knowledge Network Database,Wanfang Database,Chinese Biomedical Literature Database,Chinese Journal Full-text Database and PubMed Database,Cochrance Library Database for nearly 18 years of Chinese medicine external treatment for chronic prostatitis.According to the inclusion criteria,the randomized controlled trials of traditional Chinese medicine treatment for chronic prostatitis were strictly screened and systematically reviewed with the help of Review Manager 5.2.Results:1.Study on the rules of clinical medication(1)A total of 303 prescriptions were retrieved,involving 275 traditional Chinese medicines.The frequency of use of all medicines totaled 2510.The medicines with a frequency of more than 30 have a total of 25,in order:Phellodendron amurense,Astragalus,Dandelion,Salvia miltiorrhiza,Daxueteng,Peach kernel,rhubarb,Poria cocos,White flower Hedyotis diffusa,WangBuLiuXing,Safflower,Red peony,Zeeland,Frankincense,Curcuma,Wild Chrysanthemum,Licorice,Hagi,Corydalis,Honeysuckle,Neem,Trigonum,Myrrh,Polygonum cuspidatum,Zanthoxylum.(2)Among the medicinal properties of drugs,the cold drugs have the highest frequency,accounting for 28.9%of the total frequency.The second is the flat taste,accounting for 22.5%of the total frequency.In terms of medicinal taste,the frequency of bitterness is the highest,accounting for 42.9%of the total frequency.Followed by spicy taste,accounting for 24.7%of the total frequency.Among the medicinal meridians,the drugs that go to the liver meridian have the highest frequency,accounting for 26%of the total frequency.The second is the stomach meridian,accounting for 16.8%of the total frequency.According to the classification of drug efficacy,the frequency of heat-clearing drugs is the highest,accounting for 39.7%of the total frequency.Activating blood and removing blood stasis is the second most frequent,accounting for about 30.5%of the total frequency.2.Systematic evaluationA total of 24 articles were included,involving 2423 cases.There are 23 low-quality articles and 1 high-quality articles.According to Meta analysis,Chinese medicine external treatment may have advantages in the treatment of chronic prostatitis.The funnel chart analysis of the subgroups with more than 6 articles is not symmetrical,which may indicate publication bias.Conclusions:1.The treatment of chronic prostatitis with traditional Chinese medicine is based on syndrome differentiation,and the most commonly used drugs are heat-clearing drugs and activating blood circulation to remove blood stasis.Among them,Phellodendron chinense,Radix Astragali and Dandelion are used more frequently;Most of them are the cold drugs;in terms of the taste,the bitter,spicy,and sweet drugs are the most used;in terms of drug meridian,liver,stomach,heart,spleen and large intestine are the main drugs2.The systematic review of the literature basically confirms that there may be advantages in treating chronic prostatitis with external treatment of traditional Chinese medicine.However,due to the low quality of the included literature,publication bias,and the heterogeneity of the included literature under certain indicators,its clinical effectiveness and safety need to be further confirmed by future high-quality,multi-center,large-sample randomized controlled trials. |
