Based On Data Excavated The Medicine Application Of Traditional Chinese Medicine In The Treatment Of Gout

Objective: The prescriptions of traditional Chinese medicine for gout were collected,and the data mining method of TCM inheritance support platform was used to summarize the medication rules and prescription characteristics.Methods: The retrospective study collected cases from January 2019 to December 2020 in the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine rheumatology outpatient and inpatient treatment has been clearly diagnosed as gout,and take traditional Chinese medicine treatment of gout patients.A total of 474 effective Chinese medicine prescriptions were collected.The collected medical records meet the inclusion criteria to establish a database,review data again,eliminate missing or suspicious data,after the review,enter the ’ Traditional Chinese Medicine Inheritance Support System(V2.5)’ system,and ensure the accuracy of all data in the established database,provide guarantee for the subsequent data mining analysis process.A variety of practical data mining methods such as association rules,complex system entropy method and entropy hierarchical clustering method are used for in-depth analysis.Results: [1st] In this study,474 prescriptions of traditional Chinese medicine for gout were collected,a total of 215 drugs were used,and the cumulative frequency was 6739 times.[2nd] 36 kinds of high frequency drugs(frequency ≥ 60 times): Achyranthes bidentata,Coix seed,Atractylodes macrocephala,Poria,Glycyrrhiza uralensis,Salvia miltiorrhiza,Mianbixie,Tufuling,Chenpi,Gentiana macrophylla,Pinellia,etc.;drug statistics are water and dampness medicine,rheumatism medicine,promoting blood circulation and removing blood stasis medicine,heat medicine,qi medicine,dampness medicine,qi medicine,expectorant cough asthma medicine,relieving drugs;most drugs are warm,flat,cold,taste sweet,bitter,pungent,into the liver,spleen,stomach,lung.[3rd] The association rule calculation shows that the commonly used drug combinations are Achyranthes bidentata and Coix seed,Atractylodes macrocephala,Poria,licorice,etc.,and the core drug combination is Poria-Atractylodes-Glycyrrhiza-Coix seed-Achyranthes bidentata.The drug combinations of damp-heat accumulation syndrome include Achyranthes bidentata,Coix seed,Phellodendron amurense,Dioscorea bulbifera,Salvia miltiorrhiza,Poria cocos,Glycyrrhiza uralensis,Atractylodes macrocephala,Rhizoma atractylodis,Gentiana macrophylla and Polygonum cuspidatum.Drug combinations of spleen deficiency and dampness syndrome include atractylodes,poria cocos,coix seed,achyranthes bidentata,yam,tangerine peel,pinellia,licorice,astragalus.The drug combination of phlegm and blood stasis obstruction syndrome contains Achyranthes bidentata,Salvia miltiorrhiza,Dioscorea bulbifera,Atractylodes macrocephala,Poria coix seed,Poria cocos,Gentiana macrophylla.Drug combinations of liver and kidney deficiency syndrome include Achyranthes bidentata,Mulberry parasitic,Duhuo,Eucommia ulmoides,Gentiana macrophylla,Atractylodes,Poria,Glycyrrhiza uralensis,Chenpi,Coix seed,etc.[4th] Entropy clustering algorithm concluded that the highest correlation coefficient of drug pair is Shiwei-Qinjiao,and 14 drug core combinations including Zhuru-Sigualuo-Luoshiteng,Chenpi-Fabanxia-Huangbai,etc.On this basis,seven new prescriptions were excavated,such as Zhuru-SigualuoLuoshiteng-Zhishiteng-Zangjiang-Maopu,Chenpi-Fabanxia-HuangbaiWeishiwei-Gegen-Maizexu,etc.[5th] The pathogenesis of gout is based on the deficiency of vital energy,and the obstruction of meridians and collaterals by damp-heat turbidity and blood stasis is the standard.The treatment methods are mostly based on invigorating spleen and infiltrating dampness,removing blood stasis and dredging collaterals,and clearing heat.The commonly used drugs for the treatment of damp-heat accumulation syndrome are mostly modified by Simiao Pills.The commonly used drugs for the treatment of spleen deficiency and dampness obstruction syndrome are Shenling Baizhu Powder and Erchen Decoction.The commonly used drugs for the treatment of phlegm and blood stasis obstruction syndrome are Shuanghe Decoction.The drugs used for the treatment of liver and kidney deficiency are Duhuo Jisheng Decoction and Sijunzi Decoction.The new prescription has the effects of invigorating spleen,eliminating pathogens,clearing heat,dredging collaterals,resolving phlegm,and removing blood stasis.The addition and subtraction of syndrome differentiation and treatment in clinical practice can be used to treat gout.Conclusion: In this study,data mining methods were used to find that gout is more common in clinical practice with damp-heat accumulation syndrome,spleen deficiency and dampness syndrome and phlegm and blood stasis syndrome.High-frequency medication,related drug categories,meridian tropism and core drug combinations were explored,and the experience of invisible medication in the treatment of gout was explored,and new prescriptions for treatment were excavated to provide reference for clinical application.
