Objective: To understand the status quo of health literacy,self-management ability and disability in COPD patients and explore the correlation among them,further analyze the impact of health literacy and self-management ability on disability in COPD patients,and provide theoretical basis for clinical development of targeted intervention measures for COPD patients.Methods: Convenience sampling was used to select COPD in capacitated patients from respiratory wards of 4 grade A hospitals in Jilin Province.General data questionnaire(including dyspnea scale,respiratory disease Anxiety Scale),chronic obstructive pulmonary disease Health Literacy Scale(COPD-Q),chronic obstructive pulmonary disease Self-management Scale and LB-IADL scale were used for questionnaire survey and data collection.SPSS26.0 software was used for statistical analysis of data,the differences were considered statistically significant at the test level of α=0.05,P<0.05.Results:(1)The average score of health literacy was 6.26±2.69.There were statistically significant differences in the health literacy of the subjects in age,medical payment method,family per capita monthly income,educational level,course of disease,number of acute exacerbations within one year,comorbidity,smoking,exercise,degree of dyspnea and anxiety(P < 0.05).(2)The average score of self-management ability was 157.01±30.39.There were statistically significant differences in subjects’ self-management ability in gender,age,residence mode,spouse status,medical payment mode,family per capita monthly income,education level,disease course,acute aggravation times within one year,comorbidity,exercise status,dyspnea degree and anxiety(P < 0.05).(3)The mean disability score of subjects was 27.34±10.77.There were statistically significant differences in disability of subjects in gender,age,ethnicity,residence mode,medical payment mode,family per capita monthly income,education level,disease course,acute aggravation times within one year,comorbidity,smoking,exercise,dyspnea degree and anxiety(P < 0.05).(4)There was a moderate negative correlation between health literacy and disability(r=-0.618,P < 0.001).Self-management ability was negatively correlated with disability(r=-0.647,P < 0.001).There was a moderate positive correlation between health literacy and self-management ability(r=0.635,P < 0.001).(5)Movements of the object of study,presence of anxiety,degree of difficulty in breathing,one year,the number of exacerbations,age,health literacy,self-management ability can effectively explain the fail,all of its variables,a total of 63% of the total variance explained,including health literacy explanatory power on the rate of disability was 11%,the self management ability explanatory power on the rate of disability was4%.Conclusions:(1)The subjects’ health literacy and self-management ability were at the medium level,and the disability level was high.(2)The higher the health literacy level,the better the self-management ability,the lower the disability degree.(3)Exercise status,anxiety,dyspnea,acute exacerbations in one year,age,health literacy and self-management ability were the main influencing factors of disability. |
