BackgroundAs the population continues to age,chronic diseases have become a global health problem that threatens people’s lives and health,and morbidity and mortality from various chronic diseases are increasing rapidly.At present,patients with multi-morbidity have become the focus group of chronic disease treatment and management,and most of them need long-term medication adherence.multi-morbidity lead to polypharmacy and adverse drug reactions,which aggravate the burden of patients.However,most current studies show that chronic disease patients generally have low medication compliance,and rural patients have worse medication compliance than urban patients.Improving medication compliance of patients with multi-morbidity in rural areas,especially those with multi-morbidity,is essential for improving the quality of healthy lives of rural people and contributing to the Healthy China 2030 goal.ObjectiveMultiple drug therapy in patients with multi-morbidity in rural related investigation and study,understand the current status of rural multiple medication compliance in patients with multi-morbidity,explore the related influencing factors,so as to slow disease management for grassroots medical staff,provide evidence for effective improving medication adherence intervention,actively promote the "healthy aging" process of our country.MethodsA questionnaire survey was conducted among rural patients with multi-morbidity in Shandong Province.General information questionnaire,medication belief scale and medication compliance scale were used to investigate the basic health and medication status of patients.SPSS26.0 statistical analysis was used for descriptive analysis.Chi-square test,independent sample T test and one-way ANOVA were used to understand the differences in medication type,medication belief and medication compliance of patients with different characteristics.Pearson’s correlation analysis was used to analyse the relationship between perceptions of medication adherence and medication compliance.Multiple linear regression was used to analyze factors influencing medication adherence in patients with multi-morbidity.ResultsThis research involves 531 rural patients with multi-morbidity,with an average age of(69.5±8.2)years,ranging from 41 to 89 years,and the elderly population was the majority.Female accounted for 59.1%,male accounted for 40.9%;The patients were mainly agricultural laborers,accounting for 89.8%.The majority of patients had primary school education or below,accounting for 74.4%.(1)Patients with 2 to 6 diseases,2 and 3 diseases were the most,and the patients with multi-morbidity course of 5 to 10 years accounted for 47.8%,and the patients were mainly the elderly.The most common combination was "hypertension+diabetes"(12.1%),(2)Most patients chose to treat in village clinics(72.7%),and 53.9%of patients purchased drugs by themselves.(3)Most of the patients took western medicine(82.3%),and 24.5%took polypharmacy.The overall belief in medication was(5.87±3.34)points,and the belief in necessity of medication was higher than the belief in scruples.(4)Patients’ compliance was poor,only 37.3%of patients had good compliance,and more than 40%of patients sometimes did not pay attention to or forget to take medicine.(5)The results of multiple linear regression showed that the influential factors of patients’ medication compliance were the source of medical expenses,self-purchase of drugs,and medication belief.Conclusions and SuggestionsPatients with multi-morbidity can not be ignored,"hypertension+diabetes" combination is the most,and the course of disease is long.Most patients seek medical treatment in village clinics,and some patients buy drugs by themselves.The rate of multiple drug use was low,but the potential irrational drug use should be vigilant.Patients’ compliance is poor and needs to be improved urgently.Sources of medical expenditure,self-purchase of drugs and medication belief were the influencing factors of medication compliance.Patients who pay their own medical expenses,purchase drugs by themselves,and have poor medication beliefs have poor compliance,which should be paid more attention to in daily management.In view of the above findings related to the current status of patient compliance and the factors influencing it,corresponding recommendations are proposed:(1)Strengthen the comprehensive prevention and treatment of multi-morbidity,and realize the whole life cycle health management;(2)Enhancing medical support for patients and reducing their financial burden;(3)Play the role of family support and improve the care for patients;(4)Strengthen the health education of patients and enhance the belief in medication;(5)Implement prescription simplification and advocate the establishment of personal drug history. |
