In the social work professional internship, the author found that all the children with attention deficit hyperactivity disorder served did not take the medication as prescribed by the doctor, that is,the medication compliance mentioned in the existing research was low. This is a common phenomenon in existing research, which is not conducive to the treatment of children with attention deficit hyperactivity disorder, and also affects the development of social work services to a certain extent. In order to study the problem of low medication compliance of children with attention deficit hyperactivity disorder, this article analyzes the medication decision-making process of caregivers of children with attention deficit hyperactivity disorder. The author goes deep into the life situation of caregivers of children with attention deficit hyperactivity disorder, and explores the factors and mechanisms that affect the caregivers’ medication decisions. This study used semi-structured in-depth interviews to collect data and interviewed 11 carers of children with attention deficit hyperactivity disorder in 7 families.Through categorical analysis, combined with relevant theories of social work decision-making research, the interview data are sorted and summarized. The study found that there are three main factors that affect the medication decision of caregivers of children with attention deficit hyperactivity disorder, namely, relationship interaction, meaning interpretation, and caregivers’ cognitive development of attention deficit hyperactivity disorder and medication.First of all,the medication decision-making process of caregivers of children with attention deficit hyperactivity disorder is a process of continuous cognitive development.Through information collection, cognitive changes occur.Caregivers can make medication decisions independently based on their own values and judgments.Secondly, relationship interaction influences medication decision-making through pressure and its sharing mechanism. The core element of relationship interaction is parenting pressure, and caregivers emphasize the function of drugs to relieve parenting pressure. Finally, the interpretation of meaning is based on the expectations of caregivers for children and the future social competition.Caregivers comprehensively analyze the significance of medication decisions for children in the future, and make decisions to adapt to competition or to strike a balance. Therefore, relationship interaction, meaning interpretation, and caregiver’s cognitive development of attention deficit hyperactivity disorder and its treatment are unified in the medication decision-making process of caregiver of children with attention deficit hyperactivity disorder. The implication of this study for social work is to improve services for caregivers of children with attention deficit hyperactivity disorder and not blindly request medication compliance;understand the pressure of caregivers of children with attention deficit hyperactivity disorder from different perspectives, especially to help them cope with School’s pressure;to carry out interdisciplinary cooperation based on case management to improve the situation of caregivers of children with ADHD.
