BackgroundMedical professional title is an important means to evaluate the comprehensive ability of medical talents,and it is also a key method to motivate doctors to improve their personal ability.As the core human resource of medical institutions,clinicians are the main body responsible for the work of treating patients and saving people,and also the core personnel to promote various health reforms and policies.Therefore,the level of comprehensive ability and professional ethics of clinicians,especially those with senior professional titles,is not only a major event related to the life safety of patients,but also a major issue facing human resource management in the medical and health industry.How to evaluate doctors’ clinical ability objectively and systematically is of great significance to mobilize doctors to continuously improve their technical level.The CPC Central Committee,the Ministry of education and other relevant ministries have repeatedly issued documents related to the reform of health talent development system and mechanism,so as to promote the reform and development of medical talent evaluation system.However,the current evaluation system for senior professional titles of clinicians in Shandong Province is still difficult to fully reflect the professional skills and scientific research academic level of doctors in forest farms.Therefore,it is particularly important to build a scientific and reasonable evaluation index system of senior professional titles of doctors.PurposesBased on literature research and interviews,this paper combs the current situation of clinicians’ senior professional title evaluation in Shandong Province,and discusses the shortcomings of the professional title evaluation index system;Delphi expert consultation method was used to establish a more objective and scientific evaluation index system of senior professional titles in Shandong Province,so as to provide reference for the evaluation of senior professional titles of clinicians in Shandong Province in the future.MethodsFirstly,this study uses the method of qualitative interview,through the in-depth interview with the management personnel and clinicians of a third class hospital in Shandong Province,combined with the interpretation of the current policy documents related to the professional title evaluation in Shandong Province,to understand the views of hospital managers and doctors on the senior professional title evaluation,and analyze the current situation of the professional title evaluation of doctors in Shandong Province.Based on the results of qualitative interviews,combined with literature research and panel discussion,this paper constructs the draft index system and designs the expert consultation questionnaire.For the selected 25 experts,expert correspondence is carried out to screen and formulate the evaluation index system of senior professional titles of clinicians in tertiary public hospitals in Shandong Province.Build the hierarchy model and the importance judgment matrix of indicators at the same level,invite experts to assign scores to the judgment matrix according to the relative importance of indicators,analyze the expert scoring data with the analytic hierarchy process,and finally calculate the index weight value.In the empirical study of the index system,a questionnaire was designed according to the obtained index system,and a questionnaire survey was conducted on 210 doctors to evaluate the rationality of the index system and the respondents’ attitude towards the index system.Results(1)Through qualitative interviews,it is found that there were some deficiencies in the existing professional title evaluation index system in Shandong Province,such as quantitative difficulties,"one size fits all" evaluation standards,lack of hierarchical and classified evaluation,excessive reliance on academic papers and lack of investigation on professional ethics;(2)After two rounds of expert consultation,combined with expert opinions and scoring,we excludes seven three-level indicators in the standard,adds two three-level indicators,combines four three-level indicators,and finally forms four first-level indicators including medical level,teaching,scientific research,comprehensive quality and professional ethics,Evaluation index system of senior professional titles of clinicians in tertiary hospitals of Shandong Province with 12 secondary indicators and 38 tertiary indicators;(3)The analytic hierarchy process calculation results show that the consistency ratio CR values of the three-level indicators are less than 0.1.After the consistency test,the CR value of the consistency ratio is less than 0.1.Among them,the weight of secondary indicators such as teaching workload,academic conference,clinical workload and scientific research achievements is large;(4)Based on the constructed evaluation index system of senior professional titles of clinicians,this study designed the clinician ability Self-Evaluation Questionnaire,The Cronbach a coefficient is 0.853,which shows that the internal consistency reliability of the questionnaire is good;KMO value and Bartley test value were 0.757 and 4596.16 respectively,P<0.05,indicating that the questionnaire is suitable for factor analysis and has good structural validity.(5)According to the self-assessment scores of clinicians according to the index system of the questionnaire survey,the respondents were divided into junior professional title groups and senior professional title groups.There was significant statistical difference in the total self-assessment scores between the two groups.According to the dimension analysis of primary indicators,the differences between the two groups in the dimension of scientific research were statistically significant;The difference of some indicators in other dimensions was not statistically significant.(6)For the evaluation of the index system,Two group’s proportion of which with satisfactory scores was more than 93%,and the difference was not statistically significant;The scores of the index of rationality and availability were higher,and the difference was not statistically significant.Conclusions and SuggestionsThis study constructed a new evaluation index system for senior professional titles of doctors,and set the weight for each index.According to the questionnaire,the survey found that the new index system had good reliability and validity,and doctors had a high evaluation on the satisfaction,rationality and availability of the index system.The new index system has rich sources and traces to follow,clear index levels and easy quantitative measurement,which has certain guiding significance for the evaluation of clinical senior professional titles in Shandong Province in the future.In view of the problems existing in the evaluation of senior professional titles of doctors in Shandong Province and the construction process of evaluation index system,the following suggestions are put forward:(1)Explore multi-dimensional and multi classification evaluation to eliminate the chronic disease of "one size fits all" of professional title evaluation standard(2)The evaluation should be guided by professional and technical ability,and continue to weaken the excessive dependence on academic qualifications and papers(3)The index system will be applied to the evaluation of senior professional titles of doctors in three-level public hospitals in Shandong Province in the
