ObjectiveTo study the clinical nursing data of patients after thoracolumbar fractures,through the application of drugs on Shenque,Tianshu and Qihai points of patients,to observe the effect of acupoint application on constipation after thoracolumbar fractures.Influence of patients,and comprehensive evaluation of patients’ treatment results from multiple aspects such as bowel sounds,defecation pattern,comfort,nursing satisfaction,quality of life,etc.,To provide traditional Chinese medicine nursing intervention methods with more emphasis on prevention and systematic treatment and higher comfort for patients with thoracolumbar fractures.MethodsIn this study,92 patients who were in bed after thoracolumbar fracture surgery in Zhengzhou Orthopaedic Hospital from April 2020 to December 2021 were selected as convenience sampling.92 patients were divided into two groups,namely conrol group and observation group with 46 patients in each group by random number table method.The two groups were given the similar education,and in addition,the observation group was given acupoint application intervention nursing.The acupoint application was prepared by Rhubarb 9g,Myrcanite 6g,Shengdi 9g,Angelica 9g,Achyranthes bidens 6g,Deserticola 6g,and Rhizoma attylodis 6g which were provided by the pharmacy department of our hospital.These medicinal materials were polished into powder at high speed and then was blended into paste with vaseline to make a circular ointment with thickness of about 0.2cm and 2cm for length and width,which is now used and prepared.On the first day after the operation,the patients in the observation group were given the first acupoint application,and were applied for 8 hours every day for a week.Using SPSS21.0 software,statistical methods such as descriptive analysis,independent sample t test,and chi-square test were used for data analysis to compare the baseline data of the two groups,such as the number of bowel rumps per minute,the first postoperative defecation time(h),the Wexner constipation score scores,the Bristol stool traits score,the Kolcaba comfort Scale score and the Newcastle Nursing Service Satisfaction Scale score Comparison(NSNS),or the Quality of life score comparison(PAC-QOL).ResultsAll 96 patients in this study.In addition,there were 4 patients in the control group and the observation group being excluded from the study.In the control group,1 patient used Enema to defecate due to abdominal distension;1 patient withdrew due to personal reasons.One patient in the observation group was excluded due to the sticking falling off,and one patient withdrew due to personal reasons..Finally,there were 46 patients in every group participated in data comparison.1.Two groups baseline comparison.Two groups have no significant difference in terms of statistics in gender,age,fracture type,payment method and education level(P>0.05).2.Comparison of bowel sound frequency and first defecation time between two groups of patientsAfter intervention(7 days of application),The number of bowel sounds per minute in the control group was(2.99±0.90),and in the observation group was(4.04±1.02),the difference was statistically significant(P<0.05).there was a statistically significant difference in the number of bowel sounds per minute between the two groups(P<0.05).Most of the bowel sounds in the observation group were distributed at the normal level,indicating that the bowel sounds of the patients after the intervention were significantly improved.After the intervention,the first exhaust time in the observation group was(23.20±4.58)h,and that in the control group was(36.38±5.24)h,and the difference was statistically significant(P<0.05),indicating that the first exhaust time was significantly shortened after the intervention.The first defecation time in the observation group was(30.54±16.5)h,and the control group was(57.47±21.76)h,and P<0.05,the difference was statistically significant,indicating that the first defecation time was shortened after the intervention.3.Comparison of constipation symptoms between the two groups under the Wexner system scoreThe Wexner constipation symptom score of the observation group(8.24±1.93)was lower than that of the control group(12.16±2.27),the difference was statistically significant,P<0.001.Among them,the Wexner standard test group was significantly improved in the first defecation time(P<0.001),the number of stools(P<0.001),the difficulty of defecation(P=0.006),the degree of emptying(P=0,019),the number of defecation failures(P=0.002)),defecation time(P<0.001)scores were lower than those in the control group,and the difference was statistically significant(P<0.05).4.Comparison of the Bristol fecal volume scores between the two groupsInvestigations found that the proportion of the type BⅣ and BⅤ stool was 17.9%in the control group,and was 40.7%for observation group.Thus,it can be found that the feature of the patients’ stool after acupoint application nursing have been significantly improved.5.Comparison of the incidence of constipation between the two groups of patientsAccording to statistics,after 7 days of intervention,the incidence of constipation in the control group was 45.65%;the incidence of constipation in the observation group was 17.39%,which was important in statistics(P<0.05),indicating that acupoint application can reduce the incidence of constipation in patients.6.Comparison of the Kolcaba comfort level between the two groupsAfter intervention(7 days of application),The comfort score of the control group was(62.413±9.9187),and that of the observation group was(78.261±9.8735),which was important in statistics(P<0.05).7.Comparison of the nursing service satisfaction in Newcastle between the two groupsBefore intervention,the Newcastle nursing service satisfaction score was(39.20±2.68)in the observation group and(39.65±2.652)in the other group,showcasing no significant in statistics(P>0.05).After intervention(7 days of application),the score was(60.67±2.376)in the observation group and(40.43±2.97)in the other group,showcasing no significant in statistics(P>0.05).8.Two groups comparison of the life scores quality.There was no obvious difference before intervention betweeen the two groups from four dimensions of physiological,psychosocial,worry,satisfaction and the life quality score(P<0.05).After 7d of intervention,all dimensions and overall scores of the observation group were lower than those of the other group,with statistical significance(P<0.05).ConclusionsIn the nursing of postoperative constipation in patients with thoracolumbar fractures,this study is of great help in the improvement of constipation symptoms after intervention,especially in relieving patients with defecation difficulty,shortening the time of first defecation,shortening the duration of defecation,and improving stool characteristics.Applying compress can better promote the recovery of patients’ gastrointestinal function,assist intestinal peristalsis,improve patients’constipation symptoms,enable patients to more comfortably carry out treatment and rehabilitation exercises,improve satisfaction,and provide a basis for optimizing clinical TCM nursing programs.
