| Objective:To analyze the clinical efficacy of endoscopic hypothermic plasma resection and electric resection in the treatment of inverted papilloma of nasal cavity and paranasal sinus,so as to provide reference for clinical selection of more effective surgical methods.Methods:The patients admitted to our hospital from September 2013 to July2021 were analyzed retrospectively.A total of 92 patients with nasal inversion papilloma who were surgically treated in the department of otolaryngology in our hospital from September 2013 to July 2021 were collected.They were divided into a low temperature plasma group,35 cases,with an average age of 58.77±9.05 years old and 2 cases in T1 stage.,18 cases of T2 stage,15 cases of T3 stage.There were 57 cases in the power system group,with an average age of 57.28±10.48 years,9 cases in T1 stage,31 cases in T2 stage,and 17 cases in T3 stage.The low-temperature plasma group was operated by low-temperature plasma under nasal endoscope,and the power system group was operated by the power system under nasal endoscope.The operation time,hospital stay,intraoperative blood loss,postoperative complications and postoperative recurrence rates were compared between the two groups.Results:1.The operation time of T2 tumor in plasma group and dynamical system group was 52.50(45.25-60.00)min and 60.0(45.5-85.0)min,respectively,with no statistical difference(Z=-1.155,P=0.248).The operation time of stage T3 tumor was 75.0(65.0~120.0)min and 100.0(67.5-142.5)min,respectively,but there was no statistical difference(Z=-0.719,P=0.472).2.The hospitalization time of plasma group and power system group was 8.3±1.4 days and 8.7 ±3.1 days respectively,and there was no significant difference between the two groups(t = 0.732,P < 0.01).3.Complications occurred in 7 cases in the plasma group and 42 cases in the dynamic system group,with statistical difference(X ~2= 14.273,P < 0.001),including 4 cases of nasal hemorrhage in the plasma group and 23 cases in the dynamic system group,which were statistically significant(X ~2=8.748).There was no significant difference in sinus orifice drainage,facial swelling and pain between the two groups(P < 0.474).4.The postoperative recurrence was 2.90%(1max 35)in the plasma group and 17.5%(10max 57)in the power system group,with statistical difference(P > 0.047).5.The intraoperative blood loss was 34.77 ±1.906 ml in the plasma group and48.51 ±1.832 ml in the power system group(P < 0.0001).Conclusions : Hypothermic plasma resection under nasal endoscope in the treatment of inverted papilloma of nasal cavity and paranasal sinus has less postoperative complications,low intraoperative blood loss and postoperative recurrence rate,so it is worthy of clinical application. |
