The Study On The Relationship Of HPV And FHIT In Sinonasal Inverted Papilloma

Objectives:. Sinonasal inverted papilloma is a kind of benign sinonasal tumor, the origin is sinonansal endothelial cell,and its character is recurrence and canceration. Because the development of surgical technic especially sinonasal endoscopic surgery),the recovery rate of SNIP get very sublimation in recent years. But the cause and pathogenesis of recurrence and canceration are unknown now. Human papilloma virus has definite relation with human tumor,it mainly encroach human epithelial tissue of mucocutaneours,and produce benign and malignancy hyperplasia on human mucocutaneours. fragile histidine triad is a kind of anti-oncogene,it has close relation with environment cancerogen,and has high absence and abnormal transcription in a lot of human tumor tissue and cell line. This study apply deeply dependability to HPV infection and FHIT expression by polymerase chain reaction and Immunohistochemistry. The objective of this study is to approach the pathogenesy of recurrence and canceration of SNIP,predict the biological behaviour,and provid good cilinical treatment plan.Methods:Fifty SNIP and twenty SCC with complete clinical and pathological data were investigated after operation. Twenty normal mucous membrane of inferior turbinate concha are contrast group.All of them were preliminarily diagnosed at Tianjin Huanhu hospital from2006to2011. With cases and tumor tissues of patients above, We contrast the expression of FHIT and HPV infection in tumor tissues and control group by Immunohistochemistry and Polymerase Chain Reaction.1we analysis the clinical biological characterstics of SNIP in different clinical indices, such as recurrence,clinical stages,age.2We detect HPV16,18DNA with PCR.3We detect the expression of FHIT gene with Immunohistochemistry.4We analysis the dependability of HPV infection and FHIT gene according toage,clinical stages,recorrance and canceration.5Date was processed with the software of SPSS17.0to analysis.Results: (1) HPV infection was expressed in40of50(80%) cases of SNIP, the diffences with normal sinonasal tissues have statistic significance(p<0.01), he diffences with SCC have not statistic significance(p>0.05),and the expression are increasing with the clinical stages and the age.(2) FHIT was expressed in15of50(30%) cases of SNIP,the diffences with normal sinonasal tissues have statistic significance(p<0.01), the diffences with SCC have not statistic significance(p>0.05), and the expression is not relative to the clinical stages and the age.(3) The inactivation of FHIT and the activation of HPV in SNIP are distinct correlation(p<0.01).Conclusions:1Infection of HPV-DNA has an important effect in pathogenesis of NIP. Meanwhile a close relationship might be discovered between high risk HPV16type and HPV18type and recurrence and malignant transformation of NIP. If there are HPV infection in SNIP,we must take enlarged operation and adopt close follow-up visit to reduce recurrence and canceration.2The inactivation of FHIT is early event in pathogenesis of NIP. FHIT contribute the morbility of SNIP3The inactivation of FHIT and HPV infection may play an important role in the pathway of progression of SNIP and SCC. They may be identified as a new therapeutic target.
