| Background and ObjectiveNasal cavity or parana sinonasal inverted papilloma (NIP) is a rare benign nasal cavity or sinunasal tumor, inviting to occupy the nasal cavity tumor of 0.5-4%. characterized by aggressive nature and tendency to recur. This tumor also has a significant malignant potential. The rate of recurrence in NIP is 30-78%, and the rate of malignant change is 5-15%. It is still given attention by clinician and pathologist to explore the factor-influencing prognosis of NIP. The objective of this study is to detect the expression of p53, Ki67, CD44-V6 and to investigate their biological significance by comparing to clinic pathologic feature of these tumors. MethodsClinical and histological observation was used to study 47 cases of NIP; immunohistochemistry was used to investigate the expression of p53, CD44-V6 and Ki67 in paraffin embedded tissue sections of 28 cases of NIP which had follow-up date, 10 nasal cavity papillomas (NP) and 10 squamous cell carcinomas (SCC). The relationship between expressions of p53, CD44-V6, Ki67 and recurrence, malignancywas analyzed statistically. ResultsThis paper present 47 cases of NIP. The fundamental histopathologic feature was marked hyperplasic of covering epithelium, which extended into the underlying mucosa, forming subepithelial crypts or solid epithelial nests connected with the surface epithelium. Recurrences developed in thirteen (46.4 per cent) and malignancy occurred in three (10.7 per cent) of 28 patients. p53 and Ki67 labeling index (LI) in 25 NIP (not involve 3 SCC arising in NIP) were 0.67+/-0.44, 5.53+/-2.31; p53, Ki67 LI and expression of CD44-V6 are no significant difference between NIP and NP, and are difference between NIP and SCC. P53 Ki67 LI and the site of CD44-V6 protein positive expression were significantly related to malignant transformation of NIP. ConclusionNIP tend to recurred and malignant. Invasion to the adjacent bones are easy to lead to recurrence of the tumor; cellular atypical might serve as indices for the malignant degeneration of NIP. Ki67, p53 and CD44-V6 can be used to evaluate the cell proliferation activity, the degrees of the malignancy and differentiation. |
