Benign infantile convulsion with mild gastroenteritis(BICE)was first put forward by Morooka in 1982.In 1995,Komori et al.made a retrospective analysis of the clinical characteristics of children with BICE who had been treated,supplemented the symptoms and characteristics in terms of the time and form of convulsive episodes,and thus described the clinical characteristics of BICE in a more comprehensive and systematic way and put forward the concept.In China,Wu Jiahua et al.first reported this disease in 2002.With the gradual understanding of the characteristics of this disease by clinicians,case reports have gradually increased in recent years.It is preliminarily believed that BICE refers to the occurrence of convulsion before and after mild gastroenteritis symptoms in normally healthy children,without obvious dehydration,acid-base imbalance,electrolyte disorder and other manifestations,and can be accompanied by low fever or no fever.It is necessary to exclude other organic and metabolic diseases that may cause convulsion,and the overall prognosis is good without sequelae.BICE mostly occurs in previously healthy infants from 6 months to 3 years old.The incidence varies from region to region,but the overall incidence is low.There is no significant difference between male and female incidence,and BICE mostly occurs in autumn and winter.At present,the pathogenesis of BICE is not clear,which may be the result of infection,increase of convulsive substances in vivo,immune dysfunction,change of convulsive threshold,genetic susceptibility and gene mutation.The main manifestations are gastroenteritis symptoms accompanied by convulsion.The main manifestations of gastroenteritis symptoms are vomiting and diarrhea,mild dehydration,no obvious acid-base imbalance and electrolyte disorder,and low fever or no fever.Convulsion occurs before or after symptoms of gastroenteritis.Convulsion attack form is more comprehensive attack,duration is mostly less than 5 minutes,a course of disease can have 1 or multiple convulsions,the general condition of children with convulsions after remission is good.Mild gastroenteritis associated with infant benign convulsions,though the overall prognosis is good,but there are still have the risk of recurrence or converted to epilepsy,and recurrent convulsions easily cause nervous system damage,so this study summarized the clinical features of mild gastroenteritis with benign convulsion in infants,and analyze the related factors,to explore the influence factors of BICE.Through this study,it is expected to increase clinicians’ understanding of the clinical characteristics of the disease,improve the diagnostic accuracy,give reasonable examination,treatment and accurate judgment of prognosis,avoid over treatment,and reduce the mental and economic burden of the children and their families.By discussing the influencing factors of the occurrence of BICE,early intervention of risk factors can be carried out in clinical work to reduce the occurrence of BICE and the impact of the disease on the health of children.Objective1.By discussing the influencing factors of the occurrence excessive treatment,and reduce the mental and economic burden of children and their families.Methods1.From September 2018 to September 2020,86 children who met the diagnostic criteria of mild gastroenteritis with benign convulsion were selected as the observation group,and 50 children who met the diagnostic criteria of acute gastroenteritis were selected as the control group.Clinical data of the selected cases were collected,including general information,clinical symptoms and auxiliary examination,etc.,and all data were retrospectively analyzed to summarize the clinical characteristics of mild gastroenteritis with benign convulsion in infants.The data of the two groups were compared.After univariate analysis,there was statistical significance,and then the influencing factors were analyzed by multivariate Logistic regression.2.This study met the requirements of the hospital ethics Committee and was approved.At the same time,informed consent was obtained from family members and informed consent was signed.Results1.Clinical features of BICE group The mental state of the children in the observation group was generally normal,including 81 children with body temperature≤38.5℃,accounting for 94.19%,80 children with vomiting symptoms,accounting for 93.02%,58 children with stool times ≤3 times a day,accounting for 67.44%,most of the stool characteristics were watery stool or egg-drop soup stool.No obvious dehydration.66 cases of convulsion occurred in the 1st to 3rd day after the symptoms of gastroenteritis,accounting for 76.74%;59 cases of convulsion occurred in autumn and winter,accounting for 68.60%;73 cases(84.88%)had convulsion duration≤5min,74 cases(86.05%)had convulsion frequency ranging from 1 to 2,and 62 cases(72.09%)had generalized convulsion.Cranial MRI results were normal in 78 cases(90.70%)and video EEG results were normal in 65 cases(75.58%).2.Compared with the control group,the incidence of rotavirus infection and family history of convulsion in the observation group were 52.32%and 10.46%,and the serum zinc and iron levels were(49.17±6.16)μmol/L and(10.08±2.55)μmol/L,respectively,which were lower than those in the control group(P<0.05).Multivariate Logistic regression analysis showed that low serum zinc and iron levels,family history of convulsion and rotavirus infection were the influencing factors for the occurrence of BICE.Conclusions1.Low levels of serum zinc and iron,family history of convulsion and rotavirus infection are the influencing factors for the occurrence of BICE,which should be paid more attention to clinically.2.Clinical characteristics of acute gastroenteritis with benign convulsion in infants BICE occurs mostly in autumn and winter and is mainly manifested as gastroenteritis symptoms accompanied by convulsions.Convulsions usually occur in the first 3 days of the course of gastroenteritis,in the form of generalized attacks,and there may be one or more convulsions in a course of disease.
