Objective: Based on the experience of ancient medical books in the treatment of vertigo,this study conducted data mining and retrospective analysis on the vertigo medical cases of Professor Wang Mingjie,the famous traditional Chinese medicine and the founder of Xuanfu school in southern Sichuan(hereinafter honored as Wang Lao),based on the auxiliary platform of traditional Chinese Medicine Inheritance(v2.5),and summarized the medication law and academic thought of Wang Lao in the treatment of vertigo on the basis of objective data in the real world.Methods: Professor Wang Mingjie’s outpatient prescription sources in the Affiliated Hospital of traditional Chinese medicine of Southwest Medical University from January 2018 to April 2021 were screened.Based on Professor Wang’s first diagnosis "vertigo",after screening according to the inclusion criteria and exclusion criteria,380 cases with complete information were screened.Establish a medical case database through the clinical information collection system of the TCMISS,and carry out frequency statistics,association rule analysis,cluster analysis,complex system entropy clustering,entropy hierarchical clustering analysis and other data mining methods through the functions of "medical case analysis" and "prescription analysis" in "statistical analysis" and "data analysis",With the help of Microsoft Excel 2013,some statistical results were drawn to show the medication characteristics and prescription rules of Professor Wang Mingjie in the treatment of vertigo.Results: 380 cases of vertigo were screened out through data analysis,with a total of 414 cases(including the first visit and follow-up visit of the same patient).1.According to the analysis of general information,there were 108 male patients(28.42%)and 272 female patients(71.58%).The ratio of male to female was 1:2.52.The age characteristics of patients showed that the average age was 55.57 years old,with a median of 56 years old.The majority of patients were middle-aged and elderly.The proportion of patients aged 61-70 years was the highest,accounting for 28.16% of the total number.There were 8 TCM syndrome types,blood stasis syndrome have 127 times(30.68%),qi and blood deficiency syndrome have 108times(26.07%),qi deficiency and blood stasis syndrome have 54 times(13.04%),stagnating-up phlegm syndrome have 51 times(12.32%),liver-yang hyperactivity syndrome have 35 times(8.45%),liver and kidney yin deficiency syndrome have 18 times(4.35%),phlegm and blood stasis syndrome have 15 times(3.62%),and damp-heat blocking collaterals syndrome have 6 times(1.45%).2.according to the analysis of prescriptions,414 of the all tests involved 197 times of drug consumption,6598 times of cumulative frequency of medication,and 15.93 times of the average drug use.The drugs ranked 20 in the first place were Pueraria lobata,Radix Paeoniae Alba,Radix Astragali,Bombyx Batatas,Rhizoma Glycyrrhizae,Caulis Caulis,Caulis Spatholobi,Bupleurum,Woodlouse,Angelica,Acorus Tatarinowii,Radix Sinensis,Atractylodes Macrocephala,Codonopsis Pilosula,Notopterygium,Oysters,Banxia,Baikal Skullcap,Poria Cocos,Vaccaria Segetalis.The use frequency of the top 10 traditional Chinese medicines exceeded 60%,accounting for 47.14% of the total use frequency of all drugs.Traditional Chinese medicine was mainly used for2890 times(44.43% of the total data),1676 times(27.70% of the total data)and 1442 times(22.17% of the total data).The drug taste frequency is mainly pungent drugs and sweet drugs,accounting for 30.85% and29.95% of the total data respectively,followed by bitter drugs,accounting for 24.93% of the total data.The frequency of channel tropism of drugs is liver channel > spleen channel > stomach channel > lung channel > heart channel > kidney channel > gallbladder channel >Bladder Channel > Large Intestine Channel > small intestine channel >pericardial channel > Sanjiao channel.Based on the association rule algorithm,when the support level is more than 60% and the confidence level is more than 0.9,the core drugs can be calculated as 10 flavors:Pueraria,Radix Paeoniae Alba,astragalus,Caulis Caulis Bupleuri,bupleurum,chuanxiong,Glycyrrhiza uralensis,angelica,woodlouse worm and Bombyx batris.Correlation degree was 10,the penalty rate was2,result was 4,and 5 new combinations were evolved through entropy clustering of complex systems.They were:(1)woodlouse worm,bugbane,Pinellia ternata,turmeric and pueraria lobata.(2)Scorpion,astragalus,roasted licorice,Wang buliuxing,earthworm;(3)woodlouse worm,Bombyx Silkworm,Poria cocos,Pinellia ternata,chuanxiong,Caulis Caulis.(4)Clematis,angelica,peach kernel,Acorus tatarinowii,Wang buliuxing,turmeric;(5)Pueraria lobata,woodlouse worm,Bombyx batterii,Pinellia ternata,Ligusticum chuanxiong,Caulis Caulis,and Paeonia lactiflora Pall.Conclusion1.Wang Lao used traditional Chinese medicine to treat vertigo.Starting from the Xuan-fu theory and based on the syndrome differentiation of "wind,fire,phlegm,blood stasis and deficiency",he believed that the depression of brain collateral Xuan-fu was an important pathological link in the pathogenesis of vertigo.In the treatment,he flexibly used wind medicine to disperse and remove the mystery,and insect medicine to search and remove the mystery.2.According to the statistical results of high-frequency drugs,Professor wangmingjie’s core drugs for the treatment of vertigo are composed of wind medicine Pueraria,chuanxiong,astragalus,bupleurum,insect medicine ground beetle insect,stiff silkworm and leech.Wind medicine and insect medicine work together to open the mysterious mansion.3.Wind medicine,insect medicine and other products for the treatment of wind have a good opening effect on the Xuanfu of the whole body,which is consistent with the treatment style of "treating all diseases with wind first,and treating persistent diseases with wind".It further shows the important clinical value of Xuanfu theory,which is worth inheriting and using by future generations.4.Through data mining technology,the data and relationship network visualization display of famous and old TCM clinical experience is more modern and scientific than simply summarizing TCM treatment experience,which is conducive to direct clinical practice and provide reference and guidance for prescription screening in the next clinical trial. |
