Professor Wang Yanhui’s Diagnosis And Treatment Rules For Treating Gastric Cancer With Mixed Syndrome Of Cold And Heat As Side Effects Of Chemotherapy

Purpose:Based on the case analysis,summarize the diagnosis and treatment experience and medication rules of the tutor Professor Wang Yanhui in the treatment of the side effects of gastric cancer chemotherapy with cold and heat syndromes.Based on the tongue,pulse,symptoms,and medication characteristics,the etiology,pathogenesis,clinical manifestations,syndrome differentiation and medication of the side effects of gastric cancer chemotherapy with cold and heat syndromes Carry out detailed discussion in order to provide diagnosis and treatment ideas for clinical practice.Method:This study adopted a retrospective investigation and research method.From May 2016 to October 2020,Professor Wang Yanhui,Department of Traditional Chinese Medicine,Xiang’an Hospital,Xiamen University,was diagnosed as stage Ⅱ-Ⅳ gastric cancer and received the outpatient diagnosis.The cases of 2-6 cycles of chemotherapy were the research objects,a total of 68 cases.Use the Excel statistical chart to analyze the basic frequency of the patient’s case data,and then use the IBM SPSS Statistics 21 medical statistics software to perform cluster analysis and correlation analysis of the basic data.Result:Among the 68 cases in statistics,male patients are mostly female patients.The age of onset is concentrated in 50-59 years.Adenocarcinoma is the most common,and most of the patients are in the pathological stages of ⅢA,ⅢB,and Ⅳ.Side effects of gastric cancer chemotherapy.Symptoms of cold-heat complex syndrome are common:insomnia,fatigue,loss of appetite,dry mouth,constipation,oral ulcers,nausea and vomiting,diarrhea,etc.Tongue features of this syndrome:purple/lavender tongue,red tongue tip;greasy fur,white/yellow fur,yellow fur,thick/thin fur,thick fur mostly.Pulse characteristics:the right pulse is mainly composed of virtual pulses and thin pulses,and can be combined with Xuan pulse;the left pulse is mainly composed of xuan pulses,combined with thin pulses and virtual pulses,and floating pulses and Hua pulses can be seen.Commonly used medicines are mainly divided into 7 categories:spleen-tonifying and nourishing qi medicines,yang-stimulating medicines,clearing heat medicines,soothing liver and promoting qi medicines,tranquilizing medicines,phlegm-resolving and dampening medicines,and blood-activating medicines.Cluster analysis shows that there are 8 common drug combination categories:Group 1:Scutellaria baicalensis,dried ginger,dangshen,pinellia,poria.atractylodes.tangerine peel.Group 2:Astragalus.Bupleurum,Radix Paeoniae Rubra.Fructus Aurantii.Group 3:Jujube.ginger,licorice,magnolia officinalis.Group 4:Magnet,night vine,albizia leather,mother-ofpearl.Group 5:Alisma orientalis,cohosh,coix seed,and Yinchen.Group 6:keel,oyster,turmeric,araceae.Group 7:Salvia Miltiorrhiza,Zhebei,Red Peony Root,Achyranthes Bidentata.Group 8:Malt,Hawthorn,Divine Comedy.The effective rate was analyzed according to the degree of decline of the side effects of chemotherapy before and after the treatment,and it was concluded that the total effective rate within 14 days of treatment was 97.06%.The results of the correlation analysis showed that:①Insomnia is related to red tongue tip and right pulse deficiency;②Fatigue is related to right pulse deficiency;③Dry mouth is related to pale yellow and greasy tongue coating and left pulse string;④Constipation It is correlated with red tongue tip,less tongue coating,and narrow left pulse string.⑤Diarrhea is correlated with tongue purple and lavender tongue.⑥Heat-clearing medicine is correlated with pale yellow and thick tongue coating,right pulse deficiency,nausea and vomiting,⑦Soothing drugs are related to red tongue,symptoms of insomnia,and loss of appetite.⑧Suppressing liver and promoting qi drugs are related to yellow and greasy tongue coating and thin and weak right pulse strings.⑨Resolving blood stasis drugs are related to less tongue coating and thin and weak left pulse strings.The symptoms of loss of appetite are related.Conclusion:Professor Wang Yanhui combined the complex syndrome of cold and heat as side effects of gastric cancer chemotherapy with Li Dongyuan’s theory of Yin and fire.He used Banxia Xiexin Decoction and Buzhong Yiqi Decoction as the basic prescriptions to add or subtract medicines.The treatment principles were to invigorate the middle,relieve fire,and raise Yang.Combining the pathogenesis of tumor Qi stagnation,phlegm dampness,and blood stasis for syndrome differentiation.
