Study On Clinical Predictors Of Core Muscle Strength Training Effect In Patients With Chronic Non-Specific Low Back Pain

Objective:Core strength training is one of the main rehabilitation methods for patients with chronic nonspecific low back pain,but it is not effective for all patients with chronic nonspecific low back pain because of the complex and diverse pathogenic factors.Therefore,this study will carry out core strength training for patients with chronic non-specific low back pain.According to the individual characteristics of successful intervention,the test indexes will be analyzed and combined to help identify the subgroups of patients with positive response to core strength training and optimize the clinical treatment efficiency.Methods:Thirty three subjects with chronic nonspecific low back pain were included.Firstly,all the subjects’ information was recorded,including basic information of subjects,functional assessment questionnaire,psychological factors and musculoskeletal examination.No matter what the outcome of each index,all subjects received a total of 12 core strength training interventions for 6 weeks.According to the Chinese Oswestry disability index score,the patients were divided into the intervention success group and the intervention failure group.The indicators with significant differences between the two groups were found through statistical methods.Finally,the reserved predictive variables were combined in the regression model to identify the characteristics of patients who are most likely to benefit from strength training.Results:Among the 33 subjects,18(54.5%)subjects in the successful intervention group reached the minimum detectable difference of Chinese Oswestry disability index(decreased by more than 30%),and the remaining 15(45.5%)subjects did not reach the minimum detectable difference,so they were included in the failed intervention group.After the preliminary analysis of the two groups,four clinical predictors were finally determined,namely,male gender,Chinese simplified version of fear belief questionnaire score greater than 36.5,bilateral average modified Thomas less than 53.25 degrees and back extensor endurance less than 63.82 seconds.The results showed that without any test,the probability of successful treatment effect of chronic nonspecific low back pain patients through core strength training was 54.5%.If patients meet the four clinical predictive variables,the success rate can be increased to 90.9%.Conclusion:Gender,Fear-Avoidance Beliefs Questionnaire-Chinese score,bilateral average modified Thomas angle and back extensor endurance are the most important characteristics to ensure the success of strength training intervention.
