| [Objectiv]Polycystic ovary syndrome(polycystic ovarian syndrome,PCOS)is common to the childbearing age women of reproductive endocrine disease,easy to cause the childbearing age women of reproductive dysfunction.Rare clinical ovulation or persistent no ovulation,menstrual loose hair and amenorrhea,infertility,high androgen hematic disease,sugar,lipid metabolism disorders,such as insulin resistance(IR)and hyperinsulinemia as the main performance.Part of the patients with PCOS can be naturally conceived through therapy,stimulate ovulation conception or assisted reproductive technology,successful pregnancy(ART)however PCOS patients 12 weeks pregnancy abortion rate is as high as 30%～50%[1],especially in patients with PCOS overweight,pregnancy after natural abortion rate is significantly higher than normal weight[2].PI3K,and Akt,the Bcl-xL is an important signal transduction of apoptosis mechanism involved pathway,probably by regulating apoptosis genes or proteins play a role of inhibiting cell apoptosis,and early pregnancy in PCOS patients play an important role on the implantation and embryo development.This topic through the kidney AND spleen method to treat PCOS threatened abortion patients serum P AND blood beta HCG,FBG,FINS,HOMA IR,T,AND,DHEAS AND SHBG levels affect clinical observation,for the treatment of patients with PCOS early pregnancy threatened abortion development train of thought.Explore PCOS Akt threatened abortion decidual and nourish cells,the Bcl-xL change and the correlation of abortion,the decidua and nourishing Akt,low the Bcl-xL mRNA expression in cells may result in PCOS pregnancy threatened abortion.[Methods]Will in April 2014 to November 2015 in the first hospital affiliated to heilongjiang university of Chinese medicine outpatient service,the admission office of department of gynaecology and obstetrics outpatient clinic patients included in the standard is divided into PCOS pregnancy threatened abortion patients with normal body weight group of 30 cases(group B),overweight(34 cases)group C,the normal pregnancy without threatened abortion group(group A,30 cases).For B and C two groups of patients with kidney and spleen method,treatment of 4 weeks.Observation group after 4 weeks ago blood serum P,beta HCG,FBG,FINS,HOMA-IR、T,AND,DHEAS AND SHBG levels.Blood serum P,beta HCG by heilongjiang university of traditional Chinese medicine clinical laboratory testing,FBG,FINS,T,AND,DHEAS AND SHBG results by ELISA method.Will in April 2014 to November 2015 in heilongjiang university first affiliated hospital of traditional Chinese medicine outpatient service,the admission office of department of gynaecology and obstetrics outpatient clinic included in the standard of PCOS pregnancy failure were divided into group B(10 cases)and group C(10 cases),and the normal line voluntary abortion in early pregnancy into group A(10 cases).Rt-pcr detection method on pregnancy failure group(group B,C)of decidual Akt and nourish cells,the Bcl-xL level mRNA determined,and compared with normal pregnancy induced abortion group.All the data above in-depth analysis,through the SPSS 19.0 software.[Result]The first part:A,B,C of three groups of participants age,pregnant day no difference,comparable.C group BMI was significantly higher than A group and PCOS normal weight group(B group),the difference was statistically significant(P<0.05);A group and B group,the difference was not statistically significant(P>0.05).PCOS pregnancy threatened abortion patients before treatment patients’ serum P,B and C group blood beta HCG levels significantly lower than group A(P<0.05),no difference between B and C group(P>0.05);Treatment before the FPG,FINS,HOMA significantly better than group C-IR A and group B(P<0.05),there was no significant difference between A and B group(P>0.05);T level before treatment group B>>group A,group C,group A(P<0.05,no difference between B and C(P>0.05);Before the AND treatment level of group B>group C>group A,there are differences between three groups(P<0.05),A,B,C three groups before treatment DHEAS three groups had no difference(P>0.05);SHBG level before treatment group B and C group were less than group A(P<0.05),no difference between B and C group(P>0.05).Group B and C patients serum P after treatment,blood beta HCG levels compared with group A had no significant difference(P>0.05);After treatment of FPG,FINS,HOMA IR no difference between three groups(P>0.05);T,AND after treatment,DHEAS AND SHBG levels no difference between A,B,C three groups(P>0.05).Treatment process,due to personal reasons out of 4 cases of group A and group B in 3 cases,1 case of stillbirth among patients,patients in group C with 3 cases of abortion,stillbirth in 2 cases.The second part:A,B and C of three groups of patients age,pregnant day no difference,comparable.C group BMI was significantly higher than A group and PCOS normal weight group(B group),the difference was statistically significant(P<0.05);A group and B group,the difference was not statistically significant(P>0.05).PCOS pregnancy abortion decidual and nourish cells,the Akt、Bcl-xL group C<group A<group B,the Akt、Bcl-xL in PCOS pregnancy abortion decidual and nourish cells exist difference between three groups(P<0.05).[Conclusion]Through this clinical study,is likely to be overweight patients with PCOS in pregnancy threatened abortion AND one of the factors with the monitoring method of kidney AND spleen of TCM for PCOS threatened abortion in early pregnancy serum endocrine hormone level before AND after the treatment,the influence of the kidney AND spleen of TCM method can effectively improve the patient’s body blood serum P,beta HCG AND SHBG levels,lower levels of FBG,FINS,T,AND AND HOMA IR,is advantageous to the embryo in the intrauterine growth.Akt,the Bcl-xL mRNA in decidua and nourish cells show lower expression,may be the cause of PCOS pregnancy abortion,and overweight PCOS pregnancy abortion than normal weight PCOS pregnancy miscarriage Akt in decidua and nourish cells,the Bcl-xL mRNA expression level is lower. |
