Objective:To observe the clinical efficacy of the cluster needling of scalp acupuncture combined with cognitive training and pure cognitive training in the treatment of post-stroke mild cognitive impairment(PSMCI),and to compare the effects of the two programs regarding the scores of MoCA,MMSE and Barthel Index before and after treatment,so as to provide certain clinical evidence for the optimal selection of PSMCI treatment plan.Methods:Forty-four patients with PSMCI were randomly divided into experimental group(22 cases)and control group(22 cases).The experimental group was treated with the cluster needling of scalp acupuncture combined with cognitive training.The patients were placed in supine or sitting position,and selected the parietal,frontal and temporal areas in the Yu's scalp acupuncture zone.Five needles were inserted into the parietal area and frontal area,and three needles were inserted into the temporal area.The acupuncture direction is 1-1.5 inches backward,and the needle is twisted back,the frequency is about 200 times per minute,and all the needles are twisted for 3 minutes.Cognitive training was performed 30min minutes after needle retention.The cognitive training method refers to the section regarding the cognitive training in Chinese Expert Consensus on Cognitive Training and Occupational Therapy,and each training lasts for 30 minutes.The control group adopted the same cognitive training program as the experimental group.The frequency of treatment is once every 1-2 days,3 times a week,and lasted for 4 weeks.The MoCA,MMSE scale and Barthel index were assessed on the two groups of patients before treatment and 4 weeks after treatment,and SPSS 25.0 was used to statistically analyze the data to observe the effectiveness of the two programs in the treatment of PSMCI.Results:During the treatment process,1 case was dropped from the experimental group and 2 cases from the control group.In the end,21 cases in the experimental group and 20 cases in the control group completed the treatment.A total of 41 cases were included in the statistical analysis.1.Comparison of MoCA scores:In comparison between the intra-groups,the total MoCA scores of two groups after the treatment were higher than before treatment,and the differences were statistically significant(P<0.05).Among them,the visual space,executive function,attention,language,delayed recall and orientation in the MoCA scale of the two groups after treatment were higher than before treatment,and the differences were statistically significant(P<0.05).Compared with the control group,the total score of MoCA in the experimental group was significantly improved after treatment(P<0.05).Among them,the scores of visual space,executive function,attention,language and delayed recall in the MoCA scale in the experimental group after treatment were significantly improved compared with the control group,and the differences were statistically significant(P<0.05).2.Comparison of MMSE scores:In comparison between the intra-group,the MMSE scores of both groups after treatment were higher than before treatment,and the differences was statistically significant(P<0.05).Compared with the control group,the MMSE score of the experimental group improved significantly after treatment,and the difference was statistically significant(P<0.05).3.Comparison of Barthel Index scores:In comparison between the intra-group,the Barthel Index scores of both groups after treatment were higher than before treatment,and the differences were statistically significant(P<0.05).Compared with the control group,the Barthel Index score of the experimental group afterment improved significantly,and the difference were statistically significant(P<0.05).4.Comparison of total effective rate:The effective rate of the experimental group was 90.48%,while that of the control group was 70%,with statistical significance(P<0.05).Conclusion:1.Cluster needling of scalp acupuncture combined with cognitive training and pure cognitive training are both effective in the treatment of PSMCI,and the total clinical efficacy of cluster needling of scalp acupuncture combined with cognitive training is better than the pure cognitive training.2.Cluster needling of scalp acupuncture combined with cognitive training and pure cognitive training can improve the cognitive function and daily living ability of PSMCI patients.Both treatment programs can improve the scores of MoCA,MMSE and Barthel Index of PSMCI patients.Cluster needling of scalp acupuncture combined with cognitive training is better than the pure cognitive training in improving the scores of MoCA,MMSE and Barthel Index of PSMCI patients.3.Cluster needling of scalp acupuncture combined with cognitive training and pure cognitive training can improve the visual space and executive function,attention,language,delayed recall and orientation of patients with PSMCI.Cluster needling of scalp acupuncture combined with cognitive training is better than the pure cognitive training in improving the visual space and executive function,attention,language and delayed recall of PSMCI patients.4.In order to improve the clinical effect,PSMCI can be treated by cluster needling of scalp acupuncture combined with cognitive training.
