| Objective:1.To observe and evaluate the clinical efficacy and safety of traditional Chinese medicine ulcer oil in the treatment of acute radiation-induced esophagitis.2.Establish a rat model of acute radiation-induced esophagitis.Methods:1.The 64 patients with acute radiation-induced esophagitis who met the criteria for intake and discharge were randomly divided into two groups.The treatment group was given ulcer oil,2 ml each time,3 times a day,for 2 weeks,and the control group took oral Kangfuxin liquid,10 ml each time,3 times a day,for 2 weeks.By comparing the changes in acute radiation esophagitis grades,swallowing pain scores,weight changes,KPS score changes before and after treatment,the onset time of therapeutic drugs,and the use of analgesics during the observation period to comprehensively evaluated clinical efficacy of traditional Chinese medicine ulcer oil on acute radiation-induced esophagitis.The safety of ulcer oil was evaluated by comparing the changes in blood routine(WBC,NEUT,HGB,PLT),liver and kidney function(ALT,AST,Urea,CR)and adverse reactions between the two groups of patients before and after treatment.2.16 Wistar rats were randomly divided into 25Gy group and 30Gy group,each with 8 rats,and 6MV-X-rays were used to irradiate rat esophagus tissue at a dose of 25Gy and 30Gy respectively.Take the full-length esophagus on the 3rd,5th,7th and 14th day after irradiation,observe and compare the pathological damage degree of the esophagus of the two groups of rats under an electron microscope and score them,and observe the weight and eating of the two groups of rats after irradiation,according to the amount of change to determine the ideal model exposure dose and observation time.Results:1.There was no difference in baseline data between the two groups of patients(P>0.05),and they were comparable.After treatment,83.3%(25/30)of acute radiation esophagitis in the treatment group were graded 0~1,16.7%(5/30)of 2-3,and 59.4%(19/32)of control group were graded 0~1,grade 2~3 accounted for 40.6%(13/32),the difference between the two groups was statistically significant(P<0.05);the total effective rate in the treatment group was 86.7%,of which the significant effective rate was 53.4%,and the control group was 78.1%and 25.0%(P<0.05).The pain score after treatment(1.70±1.51)in the treatment group was significantly lower than that before treatment(6.07±1.28),and the pain score in the control group after treatment(3.16±1.80)was also significantly lower than before(6.13±1.24)(P<0.01).The pain score after treatment in the treatment group(1.70±1.51)was lower than that in the control group(3.16±1.80)(P<0.01).The body weight of the treatment group after treatment was(63.08±10.76)kg increased from(62.10±10.26)kg before treatment,and the body weight of the control group after treatment(63.37±9.91)kg also increased from(62.74±10.16)kg before treatment,and the difference was statistically significant(P<0.01).The improvement rate of KPS score in the treatment group was 80.0%higher than that of the control group(68.8%),but the difference was not statistically significant(P>0.05).The onset time of drugs in the treatment group was(6.19±1.06)days faster than that in the control group(7.32±1.18)(P<0.01).There were no significant changes in blood routine,liver and kidney function before and after treatment in the two groups,and no serious adverse reactions occurred during the observation period.2.The esophagus tissues of the two groups of rats had different degrees of radiation pathological damage after irradiation.The pathological score of esophageal damage in the 30Gy group was higher than that in the 25Gy group(P<0.05).The pathological score of esophageal injury of rats in the 30Gy dose group was higher than other observation time points on the 7th day after irradiation,and on the 7-9 days after irradiation,the daily food intake and body weight of the rats reach the lowest level,which is directly proportional to the degree of esophageal injury.Conclusion:1.Traditional Chinese medicine ulcer oil can significantly reduce the grade of acute radiation esophagitis,relieve the patient’s painful swallowing,dysphagia and other clinical symptoms,the effect is better than Kangfuxin liquid,and it has a faster effect than Kangfuxin liquid.It can also improve patient’s weight and KPS score.It has good safety and is worthy of clinical application.2.A single dose of 6MV-X-ray 30Gy was used to irradiate the esophagus to successfully establish a rat model of acute radiation esophagitis,the 7th day after irradiation is the ideal observation time. |
