ObjectiveTo verify the preventive effect of full-process personalized information support nursing on post-ICU syndrome of ICU patients transferred out of ICU,so as to provide a basis for clinical practice.MethodsThis study is a randomized,controlled clinical intervention trial.The critically ill patients who meet the inclusion and exclusion criteria in respiratory and internal medicine critical care unit(RICU + MICU)of a top three hospital in Sichuan Province from January 2020 to June 2020 were selected as the research objects.The patients were divided into control group and experimental group by using random number table and envelope assignment hiding method.There were 55 cases in the control group and 55 cases in the experimental group.The control group was given routine nursing measures of respiratory and internal medicine critical care,while the experimental group added personalized information to support nursing care on the basis of the control group.The baseline data of the patients were collected with a self-made general data questionnaire;seven days after the patients were transferred out of ICU,the cognitive function of the patients was assessed by mini-mental state examination,the psychological function was assessed by hospital anxiety and depression scale,and the physiological function was assessed by barthel index;6 months after the patients were transferred out of ICU,the quality of life of the patients was followed up by SF-36 Health Survey telephone.In this study,the test level was 0.05,and the difference was statistically significant(P<0.05).ResultsA total of 110 patients were included in this study.During the study,1 case lost contact during the follow-up in the experimental group,and 1 case died due to the change of condition during the follow-up in the control group.The rest of the cases completed the test as required.A total of 108 patients completed the trial,54 in the experimental group and 54 in the control group.1.Comparison of baseline data.There was no significant difference in age,sex,ICU hospitalization days,education level,APACHE-Ⅱscore,use of ventilator and sedation and analgesia between the two groups,and no significant difference in cognitive function,anxiety,depression and selfcare ability between the two groups(P > 0.05),the two groups were comparable.2.Comparison of physiological indexes between two groups of patients.(1)There was significant difference in cognitive function between the two groups7 days after ICU transfer(z=2.009,P =0.044);There was significant difference in anxiety status between the two groups at 7 days after ICU transfer(t=2.265,P=0.034);There was a significant difference in the depression status of the two groups 7days after transfer from ICU(z=-2.569,P =0.01);There was no significant difference in self-care ability between the two groups at 7 days after ICU transfer(t=-0.238,P=0.814);There was no significant difference in the incidence of PICS between the two groups after 7 days of ICU transfer(χ~2=1.018,P =0.313),but the incidence rate of PICS in the experimental group(29.6%)was lower than that in the control group(38.9%).(2)In the comparison of quality of life after 6 months,the difference of general health status between the two groups was statistically significant(z=-2.508,P=0.012);The difference of emotional function between the two groups was statistically significant(z=-2.119,P =0.034);There was significant difference in social function between the two groups(z=-1.976,P =0.048);There was significant difference in mental health between the two groups(t=-2.144,P =0.043),but there was no significant difference in health change,physiological function,role physical,physical pain and energy between the two groups(P>0.05).Conclusions1.The whole process of personalized information support nursing can effectively reduce the level of anxiety and depression of patients,avoid the development of more serious cognitive dysfunction,reduce the damage degree of cognitive function,and reduce the incidence of pics to a certain extent.2.The whole process of personalized information support nursing can have a positive impact on the general health status,emotional function,social function and mental health of patients with 6-month quality of life.
