| Objective:To explore the relevance between BMI and common metabolic indicators in patients who first experienced AMI and within the reperfusion time window,living in Jining and its surrounding areas.Methods:A total of 479 patients who were diagnosed as having the first attack of AMI and within the reperfusion treatment time window in the cardiac emergency department of the Affiliated Hospital of Jining Medical College from August 2019 to September 2021 were included in this research.The basic informations,measurement indicators,SUA,blood lipidspectrum and FPG were collected.Then calculate BMI,TG/HDL-C and Ty G.According to BMI=24.0kg/m~2,these research subjects was separated into two groups,and the distinctions of basic informations were compared,then the correlation between BMI and common metabolic indicators were analyzed.Stratified study was conducted in SUA and Ty G,TG/HDL-C respectively according to gender and the history of diabetes mellitus.Results:(1)Among the patients who were first diagnosed AMI,patients in overweight/obesity group were younger,the number of men was higher,the level of HDL-C was lower,however,the levels of SUA,Ty G,TG/HDL-C were higher,else serum lipids were higher,too.The level of FPG has no difference.(2)Univariate analysis of BMI and metabolic indicators:With the increase of BMI,the levels of SUA,TG,TC,LDL-C,Ty G,TG/HDL-C increased,and HDL-C decreased,but it didn’t matter with the level of FPG.(3)Use smooth curve fitting to investigate the relation between BMI and metabolic indicators:BMI showed a linear relation with SUA,TC,LDL-C,TG/HDL-C,and a curvilinear relation with TG,HDL-C,Ty G,and there was an inflection point.(4)Multivariate piecewise linear regression was be employed to analysis the relation of BMI and metabolic indicators:BMI has a positive linear correlation with SUA,TC,LDL-C,TG/HDL-C.When BMI does not exceed 30.43kg/m~2(28.07kg/m~2),BMI showed a positive correlation with TG(Ty G);When BMI does not exceed 23.53kg/m~2,BMI showed a negative correlation with HDL-C.After stratification by gender in SUA,still showed a linear positive correlation in male patients;The level of insulin resistance was stratified according to the diabetes history,and the relationship between BMI and indexs of insulin resistance were still remained.Conclusion:For patients with acute myocardial infarction diagnosed for the first time and within the time window of reperfusion treatment in Jining and its surrounding areas:BMI and SUA presented a linear positive correlation,more attention should pay to the increased risk of SUA caused by changes in BMI in male patients with AMI for the first time.The main dyslipidemia was high TG and low HDL-C,when the weight of patients is in different categories,the indicators of blood lipids that need attention should be different.With the increase of BMI,the degree of insulin resistance in patients with AMI for the first time increases slightly,but the degree of insulin resistance in patients with diabetes is almost twice that in patients without diabetes,it is necessary to monitor the blood glucose. |
