Research On The Demand And Influencing Factors Of Rural Elderly In Medical-Nursing Institutions Based On Andersen Model

To study the demand and cognitive status of the rural elderly in medical and nursing institutions,and analyze the influencing factors of demand.To explore the influence degree of predisposing,enabling,demand factors in Andersen model theory,and put forward relevant suggestions for the development of institutions in rural areas.Selecting the rural elderly in a county of Tangshan as the objects,and using the self-designed questionnaire in investigation.Describing the basic situation of the rural elderly,and screening out the influencing factors on the demand by chi square test(P<0.05).Before the multi factor analysis,constructing three binary logistic models,and obtaining the model with the best fitting degree.Eliminating the predisposing,enabling,and demand factors gradually,and constructing three regression models.Observing and comparing the change values of-2 log likelihood,Cox&Snell R~2 and Nagelkerke R~2 in three models,and analyzing the influence degree of the three factors.In total 495 questionnaires,454 questionnaires were effectively recovered with the rate of 91.7%.160 people(35.2%)had demand for this institutions.393people(86.6%)did not understand it.Single factor analysis showed that 18 variables,such as education level,occupation and marital status,affected the demand(P<0.05).Multi factor analysis showed that 11 variables were correlated with the demand;the rural elderly who were not engaged in agriculture,living alone or living with their spouses had higher demand.After excluding predisposing factors,the change values of-2 log likelihood value,Cox&Snell R~2 and Nagelkerke R~2 in the model were the largest.The demand and cognitive degree of the rural elderly is low.Many factors affected the demand of the rural elderly.In Andersen model,predisposing factor had the greatest impact on the demand.In the future,we should strengthen the publicity of the combination of medical care and nursing,increase the investment in the institutions,pay attention to the construction of talent team for the combination of medical and nursing,and accelerate the implementation of long-term nursing insurance system.Figure 5;Table 12;Reference 67...
