The Study Of Tcm Syndrome Elements Distribution Characteristics Of Premature Ovarian Insufficiency

Objective1.By retrospective analysis of modern literature related to premature ovarian failure(POF),we summed up the syndrome types of POF and the characteristics of syndrome factor distribution.2.Through a cross-sectional study of the clinical symptoms and signs of patients with early-onset ovarian dysfunction(POI),we analyzed the distribution characteristics of POI syndrome elements,and combined with the literature research on POF,we summarized the pathological nature of POI and its progression.Methods1.The articles that met the inclusion criteria for TCM syndromes of POF published from2000 to 2020 were retrieved from the online database.Then the syndrome types included in the literature were standardized and the syndrome elements were separated.After that,Excel was used to conduct statistical analysis on the distribution of syndrome types and syndrome elements.2.Patients with POI who visited the Gynecologic Outpatient Department of the First Affiliated Hospital of Guangzhou University of TCM from October 2020 to February 2021 were screened,and their symptoms and physical signs were collected.The syndrome factors of patients were diagnosed according to the syndrome differentiation factors scale(SCRS),and the distribution of syndrome factors was counted by Excel.Results1.Modern literature analysis of POF: A total of 25 eligible articles were included,and2963 cases were reported.46 syndrome types were extracted and summarized into 35 after specification,with the cumulative occurrence frequency of 142 times.The five most frequently-occurring syndrome types were liver-kidney yin deficiency,liver stagnation and kidney deficiency,kidney deficiency and blood stasis,qi and blood deficiency,and spleen and kidney deficiency in sequence,and the two syndrome types with the largest number of cases were liver stagnation and kidney deficiency,and liver and kidney yin deficiency in sequence.After splitting the standardized syndrome types,15 syndrome elements were obtained,including 4 syndrome elements of disease location type and 11 syndrome elements of disease nature type,with the cumulative occurrence frequency of 387 times.The syndrome elements of disease location with the most frequent occurrence and the largest number of cases were kidney,liver and spleen in sequence,and the syndrome elements of disease nature were qi deficiency,qi stagnation,yin deficiency,yang deficiency,blood stasis and blood deficiency in sequence.2.Clinical study of POI: A total of 96 cases were collected.After syndrome element weights were calculated,a total of six syndrome elements in disease location were collected,which were liver,kidney,uterus,spleen,mind and heart in sequence.The frequency of syndrome elements in liver,kidney and uterus was more than 50%,which was significantly higher than that of other syndrome elements.Moreover,the average diagnostic weights of the three syndrome elements in disease location were more than 100.A total of nine syndrome elements of disease nature were collected,and five syndrome elements with the frequency >50% and the average diagnosis weight > 100 were collected,in order of qi stagnation,blood deficiency,qi deficiency,yang deficiency and yin deficiency.Conclusion1.According to the modern literature research of POF,it is concluded that:(1)The five syndrome types of POF are liver and kidney yin deficiency syndrome,liver stagnation and kidney deficiency syndrome,kidney deficiency and blood stasis syndrome,qi and blood deficiency syndrome,and spleen and kidney yang deficiency syndrome,which are the common syndrome types in the syndrome differentiation of POF.(2)Liver depression and kidney deficiency syndrome and liver depression and yin deficiency syndrome are the common syndrome types of POF;(3)Kidney,liver and spleen are the common positional syndrome elements of POF;(4)Qi deficiency,qi stagnation,Yin deficiency,Yang deficiency,blood stasis and blood deficiency are the common syndromes of POF.2.The clinical studies of POI have revealed that:(1)Liver,kidney and uterus are the main syndrome factors of POI;(2)Qi stagnation,blood deficiency,qi deficiency,yang deficiency,and yin deficiency are the main syndrome elements of POI.3.At the early stage of POI disease,liver stagnation and qi stagnation are the main pathogenesis.With the development of POF,kidney deficiency becomes the most important pathogenesis.In the early development of the disease,soothing the liver and moving qi while protecting the kidney qi are conducive to blocking the disease in time.
