Objective:To evaluate the effectiveness and safety of Roujin Jianpi Zhichan Decoction based on the method of softening the liver and strengthening the spleen in the treatment of vascular parkinsonism(VP)with strong liver and weak spleen through clinical observation of curative effect.Provide new ideas for the treatment of VP with traditional Chinese medicine.Methods: Using a multi-center,randomized controlled method,72 patients with VP with strong liver and weak spleen were enrolled as research objects according to the standard.After signing the informed consent,they were randomly divided into two groups(treatment group and control group),with 36 cases in each group.The two groups were given basic treatment for the primary disease.The treatment group was treated with Roujin Jianpi Zhichan Decoction and Medopa.The control group was treated with Medopa.After enrollment and after the 4th and 8th weeks,the various parts and total scores of the Movement Disorder Society-Unified Parkinson’s Disease Rating Scale(MDS-UPDRS),the TCM Syndrome Score Scale,the exercise test measurement,the National Institutes of Health Stroke Scale(NIHSS),and record data on routine safety evaluation indicators.Results: In the treatment group before and after treatment,MDS-UPDRS,TCM syndrome score,finger timing exercise test and other indicators were tested by two related samples(P<0.05),and there was a statistical difference.The scores of NIHSS scale and the ten-meter reentry exercise test were non-parametrically tested by two related samples(P>0.05),and there was no statistical difference.In the control group,indicators such as MDS-UPDRS Ⅰ,MDS-UPDRS Ⅱ,MDS-UPDRS Ⅲ,MDS-UPDRS total score part,TCM syndrome integral,finger timing exercise test,etc.were tested by two related samples before and after treatment(P<0.05),with statistics Learning differences.MDS-UPDRS IV part,NIHSS scale score,ten-meter reentry exercise test and other indicators were tested by non-parametric tests of two related samples(P>0.05),and there was no statistical difference.After treatment,the two groups of patients were tested with non-parametric tests(P<0.05)for indicators such as MDS-UPDRS Ⅰ,MDS-UPDRS Ⅱ,MDS-UPDRS Ⅲ,MDS-UPDRS total score,and TCM syndrome score,and there was a statistical difference.MDS-UPDRS IV,NIHSS scale,finger timing exercise test,ten-meter reentry exercise test(P>0.05),there was no statistical difference.After treatment,the MDS-UPDRS score improvement rate(ie disease effective rate)of the two groups of patients was tested by non-parametric test(P<0.05),and there was a statistical difference.The effective rate of the treatment group was 85.3%,and the effective rate of the control group was43.8%.After treatment,the improvement rate of the TCM syndrome score scale(ie,the effective rate of syndromes)between the two groups of patients was tested by non-parametric tests(P<0.05),and there was a statistical difference.The effective rate of syndromes in the treatment group was 88.2%,and the effective rate in the control group was 37.5%.Conclusions:(1)"Strong liver and weak spleen" is one of the core pathogenesis of VP,and "softening the liver and invigorating the spleen method" is an effective treatment for VP with strong liver and weak spleen.(2)The Roujin Jianpi Zhichan Decoction based on the theory of softening liver and strengthening spleen therapy can significantly improve the scores and total scores of various parts of MDS-UPDRS in patients with VP on the basis of western medicine treatment,and the effect is better than basic western medicine treatment alone.(3)Roujin Jianpi Zhichan Decoction combined with western medicine basic treatment can significantly improve the TCM syndrome score of patients with VP,and the medical syndrome effective rate and marked healing rate are better than the basic western medicine treatment alone.(4)Roujin Jianpi Zhichan Decoction combined with basic western medicine treatment can improve the walking ability of the lower limbs of patients with VP to a certain extent.In summary,the Rougan Jianpi Zhichan Decoction based on the method of softening the liver and strengthening the spleen has a definite effect in the treatment of VP with weak liver and weak spleen.It provides new ideas and objective basis for the clinical treatment of VP,which is worthy of further research and promotion.
