| Objective To analyze the influence of ACL injury on patients,knee swinging symptoms,usual activities and athletic ability;Probing the effect of the intervention scheme based on the theoretical framework of “5E” rehabilitation model on the home-based rehabilitation adherence,knee swinging symptoms,usual activity ability,sports adaptability,pain control and the living quality of patients after ACLR.Method From November 2019 to July 2020,64 ACLR patients in the third-grade A hospital in Ningxia were choosed as the study target.Methods by random,two group patients were equal in amount(n=32).The control group patients was given the sports medicine department caring,the intervention group patients was given the sports medicine department caring+nursing interventions of the “5E” rehabilitation mode,which including nodochium and nosocomium.To measured the improvement of knee ROM,knee swinging symptoms,usual activity ability and sports adaptation ability,obedience with home exercise between the two groups after surgery for 1,3 and 6 months.The effect of medical pain management at rest and active was assessed between the two groups after operation for 2,14 days and 1 month.The living quality of the two group of patients was estimated at3 and 6 months after operation.To assess the application effect of nursing intervention based on “5E” rehabilitation model.Results1.the general information resultFinally,62 patients finished the intervention program,which lasted for 6months.The average age of 62 ACLR patients was(33.65±10.38)years;the whole average BMI was(25.23±3.28)kg/m2;the males injury was 48(77.42%)and the females injury was 14(22.58%);20(32.26%)patients suffered from sports injuries,6(9.68%)patients suffered from traffic injury,36(58.06%)patients suffered from accidental injury.The baseline data such as demographic characteristics and clinical data of the two groups were balanced and comparable(P>0.05).2.Intervention results2.1 pre-intervention outcomes2.1.1The average knee joint activity of the ACLR patients was(101.74±16.91),the mean score of knee instability was(46.45±20.84),the average score of daily activity ability and athletic ability was(41.71±16.46),all of them were at low level;The average score of quality of life was(55.91±15.39).2.1.2Through statistics we found,the home-based rehabilitation adherence,the knee swinging symptom,usual activities,sports adaptability,and living quality of the outcomes are no significance between the two groups(P>0.05).2.2 Post-intervention outcomes2.2.1Comparison of ROM between the two groups:The mean values of active knee bending angle of knee(ROM)measured by three times in the intervention group were all higher than that of the control group with postoperative 3 and 6 months(P<0.05).2.2.2Comparison of knee function scores between the two groups:After operation 1,3and 6 months,intervention group of the knee swinging symptoms(LKS score),usual activities and sports adaptability(IKDC score)werebetter than that of the control group(P<0.05).2.2.3Comparison of VAS between the two groups:On the postoperative 2days,the visual analogue scale of patients in both groups rised compared with preoperative owning to surgical trauma,there are no significance between the two group(P>0.05);On the postoperative 14 days and 1 month,the knee pain intensity of patients in both groups on the decline,and the VAS score during the rest and active in the intervention group was lower than that of control group(P<0.05).2.3.4Comparison of compliance between the two groups of patients with home rehabilitation exercise:After the 1,3 and 6 months of operation,the obedience rates of the control group were 68.32%,68.57% and 71.92%,and the intervention group were 73.97%,78.59% and 84.02%.The home-based exercise obedience of the intervention group was greater than those in the control group(P<0.05).2.3.5Comparison of quality of life and each dimension score between the two groups:The total score of quality of life in the intervention group(77.76±8.06)was better than the control group(64.77±9.62)at 3 months after operation,the total score of quality of life in the intervention group(85.56±8.07)was better than the control group(73.94±9.10)at 6 months after operation;The scores of physiological function dimension and bodily pain dimension in the treatment group were better than those in the control group at 3 and 6 months after operation(P<0.05);Both the physiologic functioning restrictions dimension and the public activities restrictions dimension of the intervention group were lower than those of the control group at 6 months after operation(P<0.05).Conclusions1.Patients with ACL injury have lower scores of range of motion,the knee swinging symptom,ability of daily activities and sports adaptability,their daily activities and sports are seriously affected.2.Nursing intervention based on “5E” rehabilitation mode can effectively ease their postoperative pain and improve the compliance behavior of rehabilitation exercise.3.The nursing intervention based on “5E” rehabilitation model caneffectively improve the knee function and quality of life of patients through rehabilitation guidance and monitoring during hospitalization and after discharge. |
