| Objective1.To construct medication adherence model for schizophrenia based on the theory of planned behavior,to verify applicability of the model in the field of medication compliance for schizophrenia in the field of medication compliance for schizophrenia.2.According to the intensity of the effect of each variable on the medication compliance behavior in the model to explore the main influencing factors and the path.So as to provide scientific reference for improving the medication adherence of patients.MethodsConvenience sampling 300 cases of patients who took antipsychotic drugs from October2019 to June 2020 in department of outpatient in three psychiatric hospitals in the central and western regions were selected as the research object.Using the general information questionnaire, international common questionnaires based on the Theory of planned behavior for medication adherence in patieng, including ITAQ,MARS,SQLS,and GSES were selected as research tools.SPSS 26.0 and AMOS 23.0 were used for statistical analysis of all data. The state of personal characteristics, medication attitude, subjective norm, perceived behavior control, medication intention, and medication behavior were explored by descriptive statistical methods with using mean±standard deviation, frequency, and composition ratio;using structural equation model to quantitatively analyze the path relationship of medication attitude, subjective norm, perceived behavioral control and medication intention on the schizophrenia.ResultsIn the current survey stage,300 questionnaires were distributed,300 questionnaires were valid, the recovery rate was 100.00%.274 questionnaires were valid, and the efficient rate was91.33%.A convenience sample of 274 subjective was included, with a mean age of 38.97 years(SD 12.14).Male 131 cases(47.8%),female 143 cases(52.2%).1.The average score of medication adherence behavior of schizophrenia was 2.71±1.26,and the average scores of entries in dimension from high to low were as follows: I forgot to take the medicine(0.78±0.41),I don’t care about the time of the medicine(0.74±0.44),I feel good and stop the medicine(0.62±0.49),and I feel uncomfortable and stop the medicine(0.57±0.50).The scores of other dimensions were: medication attitude score(5.86±2.13),subjective norm score(23.92±7.88),perceptual behavior control score(3.64±2.94),medication intention score(43.42±17.96).2.The results of structural equation model shows that:(1)The model based on the theory of planned behavior could explain 36% of the variance of medication adherence behavior of schizophrenia.(2)Medication intention with the highest overall effect in medication adherence, the total effect value of 0.063;subjective norm not only had a direct effect on medication behavior in schizophrenia, but also indirectly affected it through medication intention, with a total effect value of-0.073(direct effect value =-0.069,indirect effect value =-0.004);Medication attitude had an indirect effect on medication behavior in outpatients with schizophrenia through medication intention, with the total effect value of 0.041;perceptual behavior control had indirect effects on medication behavior in schizophrenia through medication intention, with a total effect value of0.023.(3)The model could explain 47% of the variance of medication intention of schizophrenia.ConclusionsThe medication adherence of schizophrenia is at a poor level and needs to be further improved. The structural equation model based on the theory of planned behavior can well explain the medication adherence of schizophrenia. That is,medication intention has a direct effect on medication adherence behavior.subjective norm not only have a direct effect on medication adherence behavior, but also have an indirect effect on medication adherence behavior through medication intention.Subjective norm, medication attitude, perceived behavioral control have indirect effect on medication adherence behavior. |
