| Objective: Introduce the method of pedicle screw internal fixation via the Wiltse approach,and compare it with the traditional posterior surgical approach.Methods: A total of 69 patients with single-segment thoracolumbar fractures without nerve injury were recruited,and two different approaches were used for pedicle screw fixation.Among them,34 cases were open surgery through the traditional posterior approach and 35 cases were through the Wiltse approach.Compare the operation time,blood loss,postoperative drainage,postoperative hospital stay,postoperative R value improvement,Cobb angle and visual analog scale(VAS)of the two methods.Results: There was no significant difference in the improvement of R value and Cobb angle between the two groups.However,compared with the traditional posterior approach open surgery method,the Wiltse approach surgery method has obvious advantages in terms of operating time,blood loss,postoperative drainage,postoperative hospital stay,and postoperative VAS score improvement.Conclusions: The Wiltse approach technology can be applied to the treatment of single-segment thoracolumbar fractures without nerve injury under the premise of ensuring the same surgical effect as the traditional posterior approach open surgery.It has small tissue trauma,short operation time and recovery time.The Wiltse approach technology has a short learning curve and is easy to popularize in hospitals at all levels. |
