Objective: Wiltse approach by comparison with the traditional midline approach open reduction and internal fixation, the clinical efficacy of thoracolumbar vertebral fractures.Methods: For July 2012- July 2013, hospital surgical treatment of thoracolumbar fractures were 62 cases,40 males and 22 females; age range of 23 to 78 years, with an average age of 45.97 years.Surgical treatment of spinal cord injury patients were graded, Frankel grade for the E-Class for compression fractures, burst fractures without neurological symptoms, and does not require the patient ’s posterior laminectomy.TRandomized patients will need surgery after the middle of the road into the traditional group and Wiltse approach group, recorded and compared two approaches required operative time, blood loss, postoperative drainage, hospitalization time, total cost of hospitalization, preoperative Cobb angle reduction surgery Cobb angle change, postoperative pain visual analog scale(visual analogue scale), and were followed up for 1 year of lower back pain disability index(Oswestry disability index, Oswestry ODI)score.Results: By Wiltse approach group in operative time, blood loss, postoperative drainage, length of stay,total cost of hospitalization, surgery in March after a year of visual pain VAS scores after 1 year Low Back Pain Disability Index ODI score after the middle of the more traditional approach has obvious advantages group, preoperative and postoperative Cobb angle Cobb angle change, according to a statistically significant difference,(P >0.05).Conclusion: In the operation indication under the premise of Wiltse approach for the treatment of thoracolumbar vertebral fracture,It has the concept of minimally invasive surgery, and it is worthy of clinical popularization for its iatrogenic injury, less operation time, less bleeding, and less complications.
