ObjectiveIn view of the current home safety risk of the disabled elderly,this paper discusses the unsafe risk events faced by the disabled elderly at home,analyzes the influencing factors of the unsafe risk events of the disabled elderly at home,strictly follows the scale compilation process,and compiles the home safety risk factor assessment scale for the disabled elderly in line with China’s cultural and medical background,so as to provide a scientific,standardized and simple way to evaluate the home safety risk of the disabled elderly Objective to provide a reliable assessment tool for the safety status of the disabled elderly.Methods1.Literature research and qualitative interview were used,this paper expounded the domestic and foreign related research on home safety,deeply explored the factors influencing the occurrence of unsafe events,and collected the perception experience of 15 disabled elderly in a tertiary class A general hospital of Wuhan from October to December in 2019,so as to understand the occurrence of unsafe events at home,and preliminarily construct the items of the risk factor assessment scale for disabled elderly at home.2.25 experts engaged in nursing and related professional work were selected to participate in Delphi expert consultation,and the first draft of the scale was optimized,and the scores and opinions of the experts on the dimensions and the importance of the items of the scale were analyzed and modified appropriately;then 350 disabled elderly people in 6communities of Wuhan were selected for home-based survey and evaluation,and the items of the scale were screened,the reliability and validity were tested to form the final version of the scale.Results1.Through consulting literature and qualitative interviews,60 kinds of related factors related to unsafe events in the elderly with disability at home were extracted,and combined with the relevant factors put forward in the qualitative study,after group discussion,Finally,37 risk factors were determined to constitute the item pool of the scale.2.In the first round,the questionnaire recovery rate was 71.42%.The average score was 2.56~4.92,the coefficient of variation was 0.06~0.39,and the overall Kendall coefficient of coordination W was 0.314.In the second round,the recovery rate was100%.The average score of each item was 4.29~4.92,the coefficient of variation was0.06~0.20,and the overall Kendall coordination coefficient W was 0.354,which were all met the standard.The authority coefficients of the two rounds of experts are 0.878 and0.903 respectively.The content validity of items ranged from 0.83 to 1,and the content validity index of total table was 0.98.Through investigation and analysis,the Cronbach’sαcoefficient of the total table is 0.932,and the Cronbach’sα reliability coefficient of each dimension is 0.944,0.944,0.954,0.944,0.955.Retest reliability was 0.987;The correlation coefficients of the total table and each dimension ranged from 0.573 to 0.788,and the correlation coefficients of each dimension ranged from 0.149 to 0.452.Five common factors were finally determined,which explained the variation of 72.375% accumulatively,and the item factor load ranged from 0.535 to 0.967.Prediction validity: When the critical value is 75.5,the prediction effect is the best,the sensitivity is 62.5%,the specificity is76.6%,and the Area under curve(AUC)is 0.731.The final scale is composed of 44 items and 5 dimensions:Physical function factors(A1~A10),affective function factors(B1~B8),family function factors(C1~C12),social function factors(D1~D7)and primary caregiver factors(E1~E7).ConclusionThe contents of this scale are relatively comprehensive,which can distinguish whether unsafe events will happen to disabled elderly people at home,and can be used as an effective assessment tool for home safety risk assessment of disabled elderly people.It can provide reference basis for the high quality of home care work and has certain practical and popularization value.
