| Objective:Analyze the current situation of research related to the prevention and control of the new crown epidemic at home and abroad,take its research hotspots and development trends as the background,construct a multi-dimensional and multifaceted hospital new crown epidemic risk assessment system and scale and empirical evidence,and make evaluation and early warning for further clarifying the risk of new crown pneumonia in different departments of the hospital,and put forward corresponding countermeasures.Methods:1.Search CNKI and Web of Science databases,collect research related to the prevention and control of the new crown epidemic in the past three years(2020-2022),and use CiteSpace 6.1.R6 and VOSviewer software to map the authors,institutions,countries,keywords,etc.included in the literature;At the same time,the mutation word map is used to display the current research hotspots to a certain extent.2.Collect literature and screen relevant items of new crown epidemic risk assessment;Use the Delphi method to evaluate articles;The normalized weight(Wi)and combined weight of each item are calculated by analytic hierarchy.3.Cronbach’s α coefficient and retest reliability coefficient were used to analyze the reliability of the scale;Content Validity 3 Index(I-CVI,S-CVI/UA,S-CVI/Ave),correlation analysis,and exploratory factor analysis exploratory scale.4.The constructed scale was used to conduct risk assessment of 38 clinical departments of a third-class general hospital in Jiangxi Province,and the TOPSIS method was used to calculate the comprehensive risk index of each department,and prevention and control suggestions were put forward for high-risk departments.Results:1.A total of 1290 Chinese literature and 2269 English literature were screened;A total of 174 authors of Chinese literature and 139 authors of English literature were analyzed.134 Chinese keywords and 82 English keywords were extracted,mainly including "Epidemic Prevention and Ccontrol","Emergency Management","Impact","Public Health",etc.Among them Chinese 6 literature produced clustering and English literature 5 were clustered,and the clustering results were significant and reasonable.2.Delphi method evaluation experts a total of 16 people,including 4 senior titles,8 deputy senior titles,4 intermediate titles,after two rounds of Delphi method scoring,showing that the authority coefficient of all experts in the two rounds is greater than 0.8,the second round is improved compared with the first round;expert opinion coordination test,P<0.05,the expert opinion coordination coefficient of the two rounds of scoring tables was effective,and the coordination of opinions was good.Finally,38 items were included,including 14 infectious sources,4 transmission routes,11 susceptible groups,and 9 coping capabilities.3.The Cronbach’s α coefficient of the scale is 0.982,the retest reliability is 0.978,both are greater than 0.7;the I-CVI of 38 items is between 0.81~1.00,all are greater than 0.78,the K*value is between 0.81~1.00,all are greater than 0.74,and the unanimous S-CVI index S-CVI/UA=0.97;the average S-CVI index S-CVI/Ave=0.97;the correlation coefficients between each dimension and the total table were 0.967 for infectious agents,0.968 for transmission routes,0.977 for susceptible populations,and 0.815 for coping capacity,P<0.05;Four components were extracted by principal component analysis,and the cumulative contribution rate of total variance variation was 94.995%,which was highly consistent with the four dimensions of the scale design.4.Comprehensive analysis of 38 departments,it is concluded that the top 10 departments in the risk index are medical imaging,pediatrics,ultrasound medicine,cardiac and macrovascular surgery,infectious diseases,emergency department,respiratory and critical care department,general medicine,otolaryngology and neck surgery,stomatology and obstetrics,in descending order.Conclusion:1.Domestic research on the epidemiology of the new crown epidemic has achieved phased results,including the effectiveness of epidemic prevention and control measures and strategies;The popularity of English research fluctuates with the development of the epidemic,and its research focuses on the treatment and prevention of drugs,and vaccines have become the focus of research.Research at home and abroad is relatively scattered,and it is necessary to strengthen the team of various research teams and enrich the research direction and content.It provides a research direction and theoretical basis for the subsequent risk assessment of the occurrence of the new crown epidemic in general hospitals.2.Construct the "Risk Assessment of New Crown Epidemic Prevention and Control in General Hospitals" scale,with a total of 38 indicators in 11 aspects in 4 dimensions;The experts evaluated the scale items well;The proportion of each dimension of the scale meets the key requirements of current epidemic prevention and control.3.The reliability and validity of the scale is good,which can be applied to the current epidemic investigation and evaluation of clinical departments,and provides a reliable assessment tool for the assessment of the risk of new crown epidemic in hospitals.4.The 38 clinical departments of the general hospital evaluated found that the risk of the auxiliary examination department was higher than that of the clinical diagnosis and treatment department;Among the clinical diagnosis and treatment departments,departments with key populations such as pediatrics,emergency departments,respiratory and critical care departments,and infectious diseases have a higher degree of risk;And put forward prevention and control opinions according to its risk characteristics. |
