| Objective :The purpose of this study is to build a medication safety assessment scale with good reliability and validity on the basis of in-depth exploration of the current situation and needs of medication safety of stroke patients at home,which can be used as an effective evaluation tool for the medication safety of stroke patients at home.Methods:Using the knowledge-attitude-behaviors theory as the theoretical basis and the five moments of medication safety proposed by the World Health Organization in2017 as the theoretical framework.A thematic group was established to construct an item pool through a large number of literature research,semi-structured interviews with doctors and patients,and brainstorming by the thematic group,forming 5 preset dimensions and 36 original item scales.Twenty-three experts were invited to conduct two rounds of expert consultation,and the boundary value method was used to screen items,forming 3 preset dimensions and 33 items of home medication safety test scale for patients.The convenience sampling method and the test scale were used to select 337 stroke patients who met the inclusion and exclusion criteria for clinical investigation.Using the method of entry options,threshold comparison method,delete item Cronbach’sα coefficient method,the entry-a score correlation coefficient method of entry for screening,screening good degree of differentiation,high sensitivity,high homogeneity,representative good entries,formed three default dimensions,brain death patients medication safety that occupy the home of 32 items the initial version of the scale.The reliability and validity of Cronbach’s alpha coefficient,retest reliability,partial reliability,content validity and structure validity were used to form three preset dimensions and 30 domestic medication safety scales for patients.Results:After item screening,three dimensions and 30 items of home medication safety scale were formed.The Cronbach’s α coefficient of the 30-item Home Drug Safety Assessment Scale for Stroke Patients was 0.886,and the Cronbach’s α coefficient of each dimension was between 0.738 to 0.822.The half-fold reliability of the total table was0.817,and the half-fold reliability of each dimension was between 0.723 and 0.753.The retest reliability of the total table was 0.993(P < 0.01),and the retest reliability of each dimension was between 0.762 to 0.871(P < 0.01).The content validity index of the home medication safety assessment scale for stroke patients was all greater than 0.75.In factor analysis,the KMO value was 0.881,and the Bartlett sphere test P<0.001.Exploratory factor analysis,The factor load values of 30 items in the home medication safety assessment scale for stroke patients were all greater than 0.4,and there was no cross with other factors.Eight common factors with eigen values greater than 1 were selected,and the cumulative variance contribution rate was 54.361%,and 8 common factors belong to3 preset dimensions;The correlation coefficient between each item and dimension and the correlation coefficient between dimension and total score were all greater than 0.4,both of which were statistically significant.Conclusion:The whole process of developing the scale follows the principle of science and preciseness,and the content of the scale is clear,concrete and objective.The home medication safety assessment scale for stroke patients has good reliability and validity and can be used to evaluate the medication safety of stroke patients. |
