ObjectiveTo develop a protective motivation intervention model based on the protective motivation theory,and to explore the effects of the protective motivation intervention model on the level of Kinesiophobia,the first time to get out of bed and the outcomes of hip joint related reports,so as to provide new ideas and theoretical basis for the nursing intervention of hip arthroplasty patients with Kinesiophobia.MethodsIn this study,92 cases of patients were selected as the research subjects,who underwent hip replacement in the orthopedics department of a third-grade class A hospital in Inner Mongolia from January to December 2020.According to the random number table method,the subjects were randomly divided into control group(46 cases)and intervention group(46cases).After operation,the control group received conventional health education,while the intervention group received health education in the mode of protective motivation intervention,according to the theory of seven variables measures,through individual education and demonstration guidance,group communication,collective education and telephone follow-up,increase patients susceptibility to disease severity and cognitive,reduce the internal and external rewards,increase the efficiency of the self-efficacy and reactionefficiacy and eventually produce protection behavior.Using general data questionnaire,Tampa Scale of Kinesiophobia(TSK)and Hip Dysfunction and Osteoarthritis Outcome Score(HOOS)evaluated,compared two groups of patients with the first time of getting out of bed after surgery,two groups of Kinesiophobia level and report outcomes at discharge and discharge a month.These outcomes include symptoms and stiffness,pain,self-care ability in daily living,exercise and recreation function and quality of life.SPSS 26.0 software was used for statistical analysis,and x2 test,t test,Mann-Whitney U test and repeated measurement analysis of variance were used to analyze the data.Results1.Before intervention,there was no significant difference in general data,TSK score and hip joint function outcome scores between the two groups(P > 0.05),which was comparable.2.Level of Kinesiophobia: The scores of Kinesiophobia in the intervention group were lower than those in the control group at discharge and one month after discharge,the difference was statistically significant(P < 0.001).The time effect and interaction effect of TSK scores in two groups were significant(P < 0.001).3.Time of getting out of bed for the first time after surgery: The time of getting out of bed for the first time in the intervention group was earlier than that in the control group,and the difference was statistically significant(P < 0.001).4.Hip function outcome:(1)The total score of hip function outcome in the intervention group was higher than that in the control group at discharge and one month after discharge,the difference was statistically significant(P < 0.001),and the time effect and interaction effect of HOOS total score in the two groups were significant(P < 0.001).(2)Symptoms,stiffness,pain and daily living ability: the scores of the intervention group were better than those of the control group at discharge and one month after discharge,the difference was statistically significant(P < 0.001),and the time effect and interaction effect of scores of each dimension in the two groups were significant(P < 0.001).(3)Sports and entertainment function dimension: there was no statistical significance in scores of patients in the two groups at discharge or one month after discharge(P > 0.05),but there was a time effect on scores of sports and entertainment function in the two groups(P < 0.001).(4)Quality of life:there was no significant difference in scores between the two groups at discharge(P > 0.05);One month after discharge,the quality of life score of the intervention group was higher than that of the control group,the difference was statistically significant(P < 0.001),and the time effect and interaction effect of the quality of life score of the two groups were significant(P< 0.001).Conclusion1.Protective motivation intervention mode can effectively reduce the level of Kinesiophobia in patients after hip replacement and promote early functional exercise and get out of bed.2.Protective motivation intervention mode can improve the self-reported outcomes of patients with Kinesiophobia after hip replacement,relieve pain and joint stiffness,improve the ability of daily life and quality of life,and make patients return to family and society as soon as possible.
