| Objective:By understanding the degree of kinesiophobia in patients with Total Hip Arthroplasty(THA),a video teach-back method intervention program for kinesiophobia in patients with unilateral total hip arthroplasty was constructed.To compare the differences between video teach-back method intervention and conventional nursing intervention on the degree of kinesiophobia,the first time out of bed,hip function,length of hospital stay and hospital costs in patients after THA.Method:A total of 91 patients with kinesiophobia after unilateral total hip arthroplasty were selected in the department of Joint Surgery of a grade Ⅲ hospital in Changchun city,Jilin Province,with 46 cases in the intervention group and 45 cases in the control group.The control group was provided with conventional care,and the intervention group was provided with video teach-back method intervention.The patients were evaluated for kinesiophobia within24 hours after surgery.If the patients met the conditions of kinesiophobia,the general data questionnaire was filled in.Discharge day Records the length of stay,hospitalization costs,and the time to first bed activity;Kinesiophobia and hip function were measured on discharge day,1 month and 3 months after surgery.Results:1.Comparing the general survey data of patients: including age,gender,BMI,religious belief,education,marital status,residence,work status and other general demographic data,as well as disease-related data such as surgical site,combined diseases and prosthesis materials.The findings indicated no statistical significance(P > 0.05),indicating comparability.2.Comparison of the degree of kinesiophobia in patients: the findings indicated that the kinesiophobia scores of the patients on the discharge date,1 month after surgery and 3 months had a time effect(P<0.001),a group effect(P<0.001)and an interaction effect(P<0.001);in the comparison between different time points,the intervention group had lower kinesiophobia scores and the difference was statistically significant(P < 0.05).3.Comparison of the first time out of bed: compared with the control group,the first time out of bed was shorter in the intervention group,and the difference was statistically significant(P < 0.05).4.Comparison of hip function: comparing the changes in hip function scores between the two groups at discharge day,1 month postoperatively,and 3 months postoperatively,the findings indicated that there were time effect(P < 0.001),grouping effect(P < 0.001)and interaction effect(P < 0.001)in the hip function scores of the two groups at different time points.The intervention group was superior to the control group at different periods of time,with statistical significance(P < 0.05).5.Comparison of hospitalization days and hospitalization costs: compared with the control group,both hospitalization days and hospitalization cost were lower in the intervention group,with the difference being statistically significant(P < 0.05).Conclusions:1.Video teach-back method can effectively reduce the degree of kinesiophobia in patients with unilateral total hip arthroplasty.2.Video teach-back method can shorten the time of first getting out of bed in patients with kinesiophobia after unilateral total hip arthroplasty,and promote the improvement of hip joint mobility.3.Video teach-back method can shorten the hospitalization days and reduce the hospitalization cost of patients with kinesiophobia after unilateral total hip arthroplasty. |
