Purpose:Collect the general data of patients with hypertension,information of four TCM diagnoses,blood uric acid level,and target organ related indexes.To observe the distribution characteristics of TCM syndromes types in hypertension patients with hyperuricemia.To explore the influence of hyperuricemia on target organs among different syndromes.To deeped the understanding of the occurrence and development of this complicated disease,it is hoped to provide evidence of TCM diagnosis and treatment and efficacy evaluation system for this complicated disease.Methods:Collect 414 hypertensive patients in the First Affiliated Hospital of Tianjin University of Chinese Medicine,between December 2019 and December 2020.According to the different levels of uric acid,they are divided into hyperuricemia group(combined group)and normal group of uric acid levels(simple group).Collect the patient’s general information,physical and chemical indicators,and TCM four diagnostic information.According to the Guiding Principles for Clinical Research of New Chinese Medicine and Internal Medicine of Traditional Chinese Medicine,syndrome differentiation type.And enter the information collected above into an Excel table,Excel establish a database,using SPSS25.0 statistical software for statistical analysis.Results:1.TCM types distribution : combined group syndrome:Phlegm-dampness syndrome(40.0%)>Yin deficiency and Yang hyperactivity syndrome(28.1%)>Yin and Yang deficiency syndrome(19.0%)>Liver fire hyperactivity syndrome(12.9%);Simple group of TCM syndrome:Yin deficiency and Yang hyperactivity syndrome(30.4%)>Yin and Yang deficiency syndrome(27.9%)>Phlegm-dampness stagnation syndrome(24.5%)>Liver fire hyperactivity syndrome(17.2%).The difference between the two groups was statistically significant(P< 0.05).phlegm and dampness Yongsheng type was the main syndrome type in the combined group,Yin deficiency and Yang hyperactivity type was the main syndromes type in the simple group.2.General information:(1)gender:Among the 210 patients included in the combined group,119 were male,accounting for 56.6% of the combined group,and 66 of the 204 patients in the simple group were male,accounting for 32.3% of the simple group.The proportion of men in the combined group was higher(P<0.01).(2)Age: The proportion of men in the combined group was higher(P<0.01).In the combined group,most males were younger than 60 years old and most females were older than 60 years old.(3)History of tobacco and alcohol: The combined group smoking(39.2%),alcohol(32.2%)were higher than the number of people accounted for the single group(P<0.01).(4)BMI: The average BMI of a consolidated group is heavier than that of a simple group(P<0.01).3.Yin deficiency and Yang hyperactivity syndrome type: The patients on this certificate are mainly male patients,The proportion of drinkers and the average BMI were also higher than that of the simple hypertension group of this type(P<0.05);To whether the high uric acid as an independent variable,target organ indicators as dependent variables,Establish multiple linear regression equations,before correcting the baseline data,the combined group’s creatinine,urine microalbumin and left ventricular mass were higher than those of the simple group(P<0.05);After adjusting gender,drinking,and BMI data,the creatinine index of the combined group was still higher than that of the simple group(β=0.32,P=0.000).4.Ying and Yang deficiency syndrome type: The patients on this certificate are mainl y male patients,The proportion of smokers and the average BMI were also higher tha n that of the simple hypertension group of this type(P <0.05);To whether the high ur ic acid as an independent variable,target organ indicators as dependent variables,Establi sh multiple linear regression equations,before correcting the baseline data,the combined group’s creatinine,urine microalbumin and left ventricular mass were higher than those of the simple group,the glomerular filtration rate was lower than that of the simple gr oup(P<0.05);After adjusting gender,smoking,and BMI data,The glomerular filtration rate of the combined group was still lower than that of the simple group(β=-0.451,P=0.000),creatinine(β=0.518,P=0.000)and urine microalbumin(β=0.348,P=0.004)w ere still high than the simple group.5.phlegm and dampness Yongsheng syndrome type:The patients on this certificate are mainly male patients,the BMI average was also higher than that of the simple hypertension group of this type(P<0.05);To whether the high uric acid as an independent variable,target organ indicators as dependent variables,Establish multiple linear regression equations,before the baseline data were adjusted,the combined group’s creatinine,urine microalbumin,and left ventricular mass were higher than those of the simple group with phlegm-dampness syndrome,and the glomerular filtration rate was lower than that of the simple group with phlegm-dampness syndrome(P<0.05);After adjusting gender and BMI data,The glomerular filtration rate of the combined group was still lower than that of the simple group(β=-0.264,P=0.002),creatinine(β=0.192,P=0.029)were still higher than the simple group.6.activity of liver fire syndrome type:The combined groups in this type of evidence are male-dominated in terms of gender(P<0.05);To whether the high uric acid as an independent variable,target organ indicators as dependent variables,Establish multiple linear regression equations,before correcting the baseline data,,the combined group’s creatinine was higher than those of the simple group(P<0.05);After adjusting gender,creatinine(β=0.192,P=0.029)was still higher than the simple group.7.There was no statistically significant difference in general information and uric acid levels among the syndrome types in the combined group(P>0.05).8.The proportion of creatinine damage of each syndrome in the combined group: Yin and Yang deficiency syndrome(45%),phlegm dampness syndrome(26.2%),Yin deficiency and Yang hyperactivity syndrome(25.4%),liver fire hyperactivity syndrome(14.8%),there are differences Statistically significant(P<0.05),Yin and Yang deficiency syndrome type accounts for the highest percentage of creatinine damage9.In terms of damage to other target organs,there was no statistically significant difference among syndrome types in the combined group(P>0.05).In conclusion:1.In the simple hypertension group,the syndrome type of yin deficiency and yang hyperactivity is the main syndrome,and the syndrome type of phlegm dampness sis the main syndrome with hyperuric acid.2.Men who smoke,drink,and have a high BMI with hypertension are more likely to have hyperuricemia3.Different syndrome types have different effects on target organ indicators when combined with hyperuricemia.When prompted to evaluate the TCM diagnosis and treatment of the comorbidity,the influential target organ indicators should be selected according to the syndrome type,make the results more accurate.4.Yin and Yang deficiency syndrome syndrome type is most affected by high uric acid:compared with the simple group,there are more target organ index types than other syndromes type;compared with the other three syndromes,the proportion of creatinine higher than the upper limit of normal is the largest.It suggests that more attention should be paid to the uric acid level of patients with deficiency of both Yin and Yang,and active intervention to reduce damage to target organs. |
