| ObjectiveTo verify the clinical efficacy of routine Western medicine treatment combined with Suzi Jiangqi decoction and Buyang Huanwu decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD),comparison was carried out on clinical symptoms, vital signs, blood gas analysis, pulmonary function and inflammatory indexes taking routine Western medicine treatment as a control,so as to provide evidence-based medicine basis for TCM treatment of AECOPD.Methods:60 AECOPD patients of lung phlegm-turbidity syndrome and pulmonary function grade Ⅱ-Ⅲ in respiratory department and inpatient department from October 2019 to September 2020 were enrolled, and divided into two groups by random number table methods, each with 30 cases.Western medicine group received routine Western medicine treatment, while TCM group received routine Western medicine treatment combined with Suzi Jiangqi decoction and Buyang Huanwu decoction. After one course of treatment, the clinical symptoms, signs, blood gas analysis, pulmonary function and inflammatory indexes(CRP,WBC,D-D)were compared between groups, and the relevant statistical data were analyzed; Then the therapeutic effect of Suzi Jiangqi Decoction and Buyang Huanwu Decoction in AECOPD was analyzed.Results1.Clinical symptoms and signs: The scores of cough, sputum, dyspnea, sibilant, anorexia and chest pain showed no significant difference between groups before treatment(P>0.05),which were decreased in both groups after treatment(P<0.05),and the decrease degree was more significant in TCM group than in Western medicine group(P<0.05).After treatment, the scores of above symptoms and signs were all lower in TCM group than in Western medicine group except chest pain(P<0.05).2.Blood gas analysis:No significant difference was found in PH,PO2,PCO2 and Sa O2 between groups before treatment(P>0.05).After treatment,PH had no significant change in both groups(P>0.05),while there was an increase in PO2 and Sa O2 levels(P<0.05),and a decrease in PCO2 level in both groups(P<0.05),and both changes were more obvious in TCM group than in Western medicine group(P<0.05).TCM group had significantly higher PO2 and Sa O2 levels,as well as lower PCO2 level than those of Western medicine group after treatment(P<0.05).3.Pulmonary function analysis:FEV1%,FEV1 and FVC levels had no difference between groups before treatment(P>0.05).Compared with those before treatment, there was an increase in FEV1% in both groups after treatment(P<0.05),and no significant changes was found in other two indicators(P>0.05).Post-treatment FEV1% and pre-to post-treatment change of FEV1%showed no significant difference between groups(P>0.05).4.Serum inflammatory factors analysis: CRP,D-D and WBC levels in serum had no significant difference between groups before treatment(P>0.05),which were decreased in both groups after treatment(P<0.05),and the decrease degree was more obvious in TCM group than in Western medicine group(P<0.05).After treatment,CRP,D-D and WBC levels in serum were lower in TCM group than in Western medicine group(P<0.05).5.Treatment efficacy analysis: The efficacy of TCM group was significantly higher than that in Western medicine group(P<0.05).ConclusioApplication of routine Western medicine treatment combined with Suzi Jiangqi decoction and Buyang Huanwu decoction can effectively ameliorate the clinical symptoms including cough, sputum and dyspnea, and the body acid-base imbalance in patients with AECOPD,which can also improve the pulmonary function, alleviate inflammatory reaction and blood hypercoagulability with no obvious side effects,so it is worthy of clinical promotion. |
