| Objective:In this study,clinical symptoms,renal function,hs-C-reactive protein,nutritional status and other related indicators were observed in patients with CKD stage 3-4.To monitor the safety and therapeutic efficacy of colon dialysis combined with Bupi Yishen Huoxue Method in the treatment of microinflammatory state and nutritional status of CKD stage 3-4 patients,so as to provide TCM solutions for clinical treatment of non-dialysis patients with CKD and prolong the process of renal replacement therapy for patients.Methods:A total of 66 patients meeting the inclusion criteria were selected from the outpatient and inpatient departments of the nephrology department of Jiangxi Provincial Hospital of Traditional Chinese Medicine on August 1,2019 and January 31,2021.According to the random number table method,they were divided into 33 cases per group(control group and treatment group).The control group was treated with modern medical treatment,and the treatment group was treated with colon dialysis and Bupi Yishen Huoxue Method.Colon dialysis was treated every other day,and the decoction for Bupi Yishen Huoxue Decoction was taken with one dose per day and two warm doses in the morning and evening.The patients were treated continuously and observed for 2 courses(1 course for 4 weeks).Clinical symptoms of patients were recorded and scored according to the standard.Microinflammation,nutritional status and renal function(hs-CRP,TP,TC,TG,TRF,ALB,HB,UA,Scr,BUN,e GFR)of patients in the two groups before and after intervention were monitored and recorded.All the data were tabulated into SPSS26.0 analysis software using Excel2007,and the corresponding statistical analysis was conducted according to the data categories to evaluate the clinical treatment efficacy.Results:(1)Basic indicators: This topic research is divided into two groups,conform to the requirements of the patients,33 cases fall off during treatment control group(3 cases),treatment group fall off in 1 case,eventually into 32 cases in treatment group,control group included 30 cases,two groups of patients with gender,age,weight,CKD stage before distribution and treatment of Chinese medicine symptom integral,related parameters of renal function and micro inflammation related indicators,calculating and analyzing the statistical indicators related to nutritional status(P > 0.05),shows that two groups had no significant differences in baseline.(2)Efficacy: the total effective rate of the treatment group was81.25%;The total effective rate of control group was 60.00%.According to the rank sum test,Z=-3.173511,P=0.001506,P < 0.05,there was a significant difference between the two groups,indicating that the efficacy of the control group was lower than that of the treatment group.(3)The curative effect of TCM syndrome and TCM symptom score: After intervention treatment,the curative effect of TCM syndrome in the treatment group was 84.38%;The control group was 63.33%.By statistical rank sum test analysis,Z=-1.999435,P=0.045561,P < 0.05,the difference was significant.The total scores of TCM syndromes in the two groups decreased after treatment(P < 0.01),but the symptom reduction in the treatment group was significantly better than that in the control group(P < 0.01).In terms of TCM symptoms,the two groups improved the main and secondary symptoms of Pishen Qixu and Yuxue Zuzhi(P < 0.01),and the treatment group was better than the control group in the improvement degree of abdominal distention,shortness of breath and laziness to speak,poor appetite,lumbago and numbness of limbs(P < 0.05).(4)Renal function: After intervention treatment,UA,SCR and BUN decreased and EGFR increased in both groups,indicating the effectiveness of treatment(P < 0.01).The improvement degree of SCR and EGFR in the treatment group was significantly higher than that in the control group(P < 0.01),indicating that the improvement effect of renal function in the treatment group was better than that in the control group.(5)The reduction of hs-CRP in the treatment group was significantly greater than that in the control group,and statistical analysis showed that the improvement of microinflammatory state in the treatment group was better than that in the control group,P < 0.05.(6)Nutritional status indicators: After treatment,TP,ALB,TC and TRF were improved in both groups(P < 0.01),and HB was also improved in the control group,but the difference was not significant(P > 0.05),and the effect of the treatment group on TG was also small(P BBB>05).After treatment,the therapeutic effect of TP,ALB and HB in the treatment group was significantly different from that in the control group(P < 0.05).The therapeutic effect of TP,ALB and HB in the treatment group was better than that in the control group,but there was no significant difference in the therapeutic effect of TC,TG and TRF.Conclusions: This study by observing the colon dialysis Combined with Bupi Yishen Huoxue Method and pure western medicine treatment of 66 patients with CKD3-4,compare the two groups before and after treatment the total effective rate,Chinese medicine symptom integral and renal function,e GFR,hs-CRP,nutrition index(Hb,ALB、TRF、 TP,TC,TG)change,prove the colon dialysis for the joint of strengthening spleen and benefiting kidney invigorate the circulation of the efficacy and safety of treatments for CKD.The results showed that colon dialysis combined with Bupi Yishen Huoxue Method in improving renal function,micro inflammation and nutritional status of CKD stage 3-4 patients,the treatment group was significantly better than the control group.It has been proved that colon dialysis combined with Bupi Yishen Huoxue Method in the treatment of non-dialysis patients with CKD is not only safe and effective,it can not only improve the clinical symptoms of patients,but also have a high efficiency of toxin removal in vivo,and there is no adverse reaction or aggravation of the condition in this study.This study provides a new treatment scheme for the clinical treatment of non-dialysis patients with CKD,which is worthy of further study and promotion,and contributes the strength of traditional Chinese medicine for the majority of patients with kidney disease. |
