Effect Of Colon Dialysis Combined With Yi Shen Decoction On Serum IS And PCS In CKD Stage 3-4 Patients

Objective:To explore the effect and significance of colon dialysis combined with Yi Shen Decoction on serum indoleol sulfate(IS)and sulfuric acid p-cresol(PCS)in chronic kidney disease(chronic kidney disease,CKD)3-4 stage patients.Methods:93 patients with stage CKD3-4 in our department from March 2018 to July 2019 were randomly divided into control group,colonic dialysis group and Yishen decoction trans colonic dialysis group.Colonic dialysis group was treated with colonic dialysis 3 times a week,2h/ times,and colonic dialysis group was treated with colonic dialysis 3 times a week on the basis of colonic dialysis,and Yishen decoction trans colonic dialysis group was treated with Yishen decoction for 3 times a week.Yishen decoction through colon dialysis group on the basis of colon dialysis group combined with traditional Chinese medicine(Yishen decoction)retention enema,the treatment observation period was 4 weeks.The changes of clinical symptoms,IS,PCS,creatinine(Scr),urea(BUN),uric acid(UA),hemoglobin(Hb),albumin(Alb),electrolytes(K +,Ca2+,P3-,Mg2+,Na+,Cl-)were observed before and after treatment.Results:1.Comparison of clinical symptoms,experimental indexes of each group(1)there was no significant difference in the levels of BUN,Scr,UA,IS,PCS,Alb,Hb,K +,Ca2+,P3-,Mg2+,Na+ and Cl-among the three groups before treatment.(2)comparison of all indexes in the same group before and after treatment: compared with those before treatment,BUN,Scr and UA in the control group decreased significantly after treatment,while BUN,Scr,UA,IS and PCS decreased in the colon group after treatment.Compared with those before treatment,BUN,Scr,UA,IS and PCS in colon dialysis group decreased significantly after treatment(P < 0.01),but there was no significant difference in other indexes(P > 0.05).Compared with those before treatment,BUN,Scr,UA,IS and PCS in Yishen decoction + colon dialysis group decreased significantly after treatment(P < 0.01),but there was no significant difference in other indexes(P > 0.05).(3)comparison of various indexes among the three groups after treatment:compared with the control group,BUN,Scr,UA,IS and PCS in colon dialysis group and Yishen decoction group decreased significantly after treatment,but there was no significant difference in other indexes.Compared with the colon dialysis group,the BUN,Scr,UA,IS and PCS of Yishen decoction after treatment in the colon group were significantly lower than those in the colon dialysis group(P < 0 05),but there was no significant difference in other indexes(P > 0 05).(4)Comparison of clinical symptoms of traditional Chinese medicine before and after treatment among the three groups.The results showed that:(1)by single factor variance test,the TCM symptom scores of the three groups were compared before treatment,P > 0.05,indicating that the TCM symptom scores of the three groups had no significant difference and were comparable.(2)after treatment,the total symptom scores of the three groups were significantly improved as compared with those of the same group(P < 0.01).However,there was no significant difference in the total symptom scores among the three groups after treatment(P > 0.05).(3)except for sore waist and knees,cold limbs,bitter mouth,nausea and vomiting and dry stool,there was no significant improvement in the control group(P > 0.05),but the other symptoms were significantly improved(P < 0.05).(4)except for sore waist andknee,nausea and vomiting,epigastric distension and soft stool,other symptoms were significantly improved in colon group(P <0.05).(5)the symptoms of Yishen decoction colon group were significantly improved except lumbar sore knees,nausea and vomiting and soft stool(P > 0.05).(6)Compared with the control group,the other symptoms in the colon group and the Yishentang colon group were significantly improved(P < 0.05),except for fatigue,weakness of waist,chilly limbs,lack of appetite,nausea and vomiting,fullness of abdominal distention,unsubstantial stool and edema(P >0.05);(7)after treatment,only dry stool was significantly improved in Yishen decoction group compared with colon group(P < 0.05),but there was no significant improvement in other symptoms(P > 0.05).2.Correlation analysis:IS was positively correlated with Scr(ritual 0.563,P < 0.001)and BUN(ritual 0.215,0.215 and 0.010),and PCS was positively correlated with Scr,BUN(0.199,0.199 and 0.020).ConclusionYishen decoction via colonic dialysis can significantly reduce the CVD-related proteins IS and PCS in patients with CKD3~4.
